About this file

This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form for RFP 269-2026-123. The form requires bidders, contractors, or subcontractors to certify that neither they nor their principals (officers, directors, managers, or entities with >10% equity interest) are currently debarred, suspended, or excluded from public transactions by federal or state agencies.

The certification specifically requires attestation that within the past three years, the bidding entity has not been convicted of fraud, criminal offenses related to public transactions, anti-trust violations, embezzlement, theft, forgery, bribery, falsification of records, making false statements, or receiving stolen property. The form provides two signature lines - one for certification and another for indicating an inability to certify, which would require an attached explanation. A false statement on this certification may result in proposal rejection, contract termination, or potential criminal prosecution.
