About this file

This is a Debarment Certification form required for submission with a Request for Qualifications (RFQ) issued by a Florida county for 82nd Avenue Utility Corridor Consulting Services in the Project Development Phase. The project involves pre-design planning, coordination, and project development for water and wastewater service expansion along the 82nd Avenue corridor, with subsequent tasks anticipated during design and construction phases. The certification must be completed and signed by an authorized official of the contractor submitting a proposal. Notably, the solicitation explicitly prohibits all contact with county staff, officials, or advisors except for the Procurement department, with violations potentially resulting in disqualification; all questions must be submitted through the Public Q & A area in Euna Bonfire.

The debarment certification requires the contractor to certify that neither the contractor nor its principals are presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from federal participation. Contractors unable to certify to these statements must attach an explanation to their RFP submission. Additionally, contractors must certify that no member or agent has had access to information that could provide an unfair competitive advantage to the firm. The form requires the signature, name, and title of the proposer's authorized official and the date of submission.
