About this file
This is Form 1 - Execution of Bid from the Texas Workforce Commission (TWC) for Solicitation 3202600161 SA Assistive Technology Home Modification. The form is a mandatory submission document that requires bidders to acknowledge receipt of all TWC Terms and Conditions, confirm agreement to provide the items and services described in the Invitation for Bid (IFB), and commit to abiding by all specified terms. Bidders must complete their organizational information including name, mailing address, billing address, phone number, email address, Employer Identification Number (EIN), and Texas Identification Number (TIN). The bid submission requires identification of a point of contact for the proposal phase and a separate point of contact for contract management. Any exceptions to the stated terms and conditions must be identified in detail and submitted with the bid; exceptions not identified at the time of submission will not be considered.
The form includes several bidder certifications and affirmations required for contract award consideration. Bidders must affirm compliance with subcontractor requirements as outlined in IFB Section 3.19 and must notify TWC of any substantial changes in financial condition, ownership, or control within specified timeframes (10 days for financial changes, 30 days for ownership changes). Additionally, bidders must provide 30 days' notice of any pending litigation, investigations, or adverse actions that may impact contract performance. The form provides multiple Texas preference categories for bidders to claim, including service-disabled veteran ownership, Texas production or procurement, agricultural products, and products meeting environmental or sustainability standards. All bidders must obtain an EIN prior to contract award, with resources provided for EIN application and TIN registration through the Texas Comptroller of Public Accounts.
