About this file
This document is a Drug-Free Workplace Affidavit that outlines six key requirements for a responding firm to maintain a drug-free workplace environment. The affidavit requires the firm to publish a statement prohibiting unlawful manufacture, distribution, dispensing, possession, or use of controlled substances in the workplace, and to specify actions that will be taken against employees who violate this prohibition.
The document mandates that the firm inform employees about drug abuse dangers, provide information about drug counseling and assistance programs, and distribute a drug-free workplace statement to all employees involved in the proposed contractual services. Additionally, employees must agree to notify their employer within five days of any conviction related to controlled substance laws, and the firm must impose sanctions or require participation in drug rehabilitation programs for employees convicted of such violations. The overall goal is to make a good faith effort to maintain a drug-free workplace through these comprehensive steps.
