PERFORMANCE WORK STATEMENT
FOR
INSULATION INSTALLATION SERVICES
1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, and labor necessary for the installation of various types of insulation materials on a wide range of applications at Dover AFB in a satisfactory manner.
1.1. BASIC SERVICES. The contractor shall install various types of insulation materials on a wide range of applications and for one or more reasons.
1.1.1. Reasons: The contractor shall install various types of insulating materials for the following reasons, energy preservation, condensation prevention, corrosion resistance/prevention, noise reduction, and safety (burn hazards).
1.1.2. Applications: The contractor shall install various types of insulating materials for the following applications, chilled water systems, domestic hot and cold water, recirculation, steam (high and low pressure), chemical pipes, air conditioning duct and equipment, mufflers, exhaust hoods and ducts, exhaust stacks, tanks air separators, chillers, boilers, drains, converters, and pumps.
1.1.3. Types of materials: The contractor is required to have the knowledge to install various types of insulation products as listed: Armaflex(sheet and pipe covering), Calcium
Silicate(block and pipe covering), Fiberglass pipe covering with all service jacket, Fiberglass duct board( ASJ and FSK finish), Mineral wool board (FSK and unfaced)
Foamglas (block and pipe covering, unjacketed and with Pittwrap), Tubolit pipe covering, Temp Mat, Fiberglas tank wrap, Aluminum jacket(smooth, corrugated, and stucco finish), PVC jacket, Bands and clips (stainless steel and aluminum), Washers (spin and cap), Nail hangers (self-adhering and glue-on, various lengths), Tapes( ASJ, FSK, Armaflex, Zeston), Armaflex glue, Armaflex finish, Rubber based weatherproof mastic (white and gray), Asphalt based weatherproof mastic (black), Insulating cement, Membrane cloth, Canvas
(various weights), Fitting covers (PVC and aluminum)
1.2. Perform all work in strict accordance with the highest safety standards and applicable codes.
2. HAZMAT REQUIREMENTS: The contractor shall submit Material Safety Data Sheets
(MSDS) of all chemicals planned for usage on Dover AFB.
2.1. Purpose. As required by AFI 32-7086, paragraphs 2.5.5.2.4, 2.5.5.2.8 and 2.5.5.2.9;
requires Dover AFB to account for all hazardous materials (HAZMATs) brought on the facility to include contractors as well as providing procedures for the reporting of those items.
2.2. This part outlines actions to be taken by the contractor in the request for approval of hazardous materials prior to initiating field activities associated with the subject project and the reporting of approved hazardous materials usage during contract performance.
2.3. Compliance with this part is mandatory and contractor failure to comply will result in the contractor not being allowed to bring any hazardous materials on Dover AFB. Contractors must also comply with mandatory Federal, State, and local guidelines, regarding storage and management of hazardous materials, failure to comply can result in legal action by Federal, State or local enforcement officers against the offender.
2.4. RESPONSIBILITIES: Contractor Reported Usage of HAZMAT.
2.5. Contractors must submit to the associated Contracting Office the information and supporting documentation (including Material Safety Data Sheets (MSDSs)) necessary to obtain HAZMAT usage authorization. Report data on the HAZMAT used during the performance of the contract at intervals and in the format specified by Dover AFB.
2.6. The contractor shall provide a completed Dover AFB HAZMAT Chemical Worksheet for each HAZMAT to be used during the duration of the contract to include MSDS. This list of all
HAZMATs shall be provided at the beginning of the contract. Any additional HAZMATs required during the course of the contract shall be requested for approval at that time. In addition, the quantity of chemicals expended will be reported to the Dover AFB Hazardous
Materials Pharmacy (HAZMART) by filling out the Contractor’s Monthly Report for
HAZMATS. Both the Worksheet and Monthly Report forms shall be provided upon award of contract. (Appendices A and B).
2.7. WASTE and HAZARDOUS WASTE REMOVAL: The contractor shall dispose of all wastes generated in the performance of their task. No hazardous waste or solid waste materials shall be placed in any trash receptacle on Dover AFB. The Contractor must comply with all federal, state, and local regulations regarding the transport and disposal of any and all hazardous wastes generated as a result of kitchen hood and duct cleaning operations under this contract.
3. GENERAL INFORMATION.
3.1. GOVERNMENT REMEDIES. The Contracting Officer (CO) shall enforce FAR 52.212.4, Contract Terms and Conditions-Commercial Items, for the Contractor’s failure to perform satisfactory services or failure to correct non-conforming services. In addition, the Contractor may be subject to contractual sanctions including, but not limited to, termination, suspension, debarment and adverse past performance assessment.
3.2. HOURS OF OPERATION. The normal hours of operation for the Contractor to perform the services required under this contract are 7:00 AM to 4:00 PM, Monday through
Friday. The Contractor is not required to provide normal service except for emergencies on the following days: New Year’s Day, Martin Luther King’s Birthday, President’s Day, Emancipation
Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving
Day, and Christmas Day.
3.3. CONTRACTOR PERSONNEL. The Contractor and their employees shall comply with all base regulations at all times and shall conduct themselves in a professional manner. Uniforms, badges, and good behavioral conduct will be mandatory.
3.4. CONTRACTOR EMPLOYEE UNIFORMS AND IDENTIFICATION. All Contractor employees shall wear a distinctive company uniform at all times while performing services under this contract. The Contractor employee uniforms shall be neat and professional in appearance and meet current commercial standards. At a minimum, all Contractor employees will wear a shirt with company logo during warm weather and a heavy outer garment with company logo during colder weather.
3.5. AIR FORCE TOBACCO POLICY. The Contractor shall ensure that all Contractor employees do not use tobacco products within any base facility, around fuel tanks and generators, within any clearly marked “No Smoking” areas or within any flight line aircraft parking areas.
3.6. RECORDS MANAGEMENT. The contractor understands that when creating, handling and maintaining records for the Air Force, either electronic or paper, you must meet the requirements established in AFRIMS Records Disposition Schedule (RDS), AFI 33-322, Records Management
Program. In order to meet the requirement established in accordance with the above-mentioned regulations, contact the Base Records Management Office (436 CS/SCXK, Bldg. 310) at 302-677-
5157 for guidance and training.
3.7. PERFORMANCE OF SERVICES DURING CRISIS DECLARED BY THE
NATIONAL COMMAND AUTHORITY OR THE LOCAL INSTALLTION
COMMANDER. This service does not meet the criteria for continuation of performance during a crisis in accordance with Department of Defense Instruction (DODI) 3020.
4. SECURITY
4.1. SECURITY INSTRUCTIONS
a. All personnel employed by the contractor in the performance of this contract, or any representative of the contractor entering the Installation shall abide by all security instructions and directives of Dover Air Force Base.
b. At the close of each work period, government facilities, equipment and materials shall be secured, lights, heat and water turned off and all doors and window secured.
c. The contractor and, as applicable, subcontractor shall not employ persons for work on this contract if such employee is identified as a potential threat to the health, safety, security, general wellbeing or operational mission of the Installation and its population, nor shall the contractor or subcontractor employ persons under this contract who have an outstanding criminal warrant as identified during the Criminal Background Check (CBI).
d. Contractors shall ensure their employees and those of their subcontracts have the proper credentials allowing them to work in the United States. Employees and subcontractors later found to be undocumented workers will be remanded to the proper authorities.
e. All contractors and subcontractors when working in a Controlled, Restricted, or other sensitive area must be escorted at all times. The military agency or unit responsible for the project is responsible for providing the escorts. The Contractor shall follow existing procedures and instructions for obtaining entrance to restricted or controlled areas
4.2 CONTRACTOR BADGE POLICIES:
All requests for Contractor badges will be submitted by a “Sponsor.” Typically, this will be the
Base Contracting Squadron (436 CONS), a Contracting Officer Representative (COR) or the
Requirement Owner (RO). The “Sponsor” will complete an Operational Risk Management
Assessment Form (ORM) and submit to the 436 SFS, Pass and Registration office for approval
IAW OPlan 31-101. As a minimum the ORM will be submitted 10- days prior to the requested date of employment.
a. All Contract employees granted unescorted access, to include Access Control Lists (ACLs), are required to consent to a Criminal Background Investigation (CBI) prior to entry to the
Installation. Continued employment is contingent upon successful completion and favorable reporting of the CBI. The Contractor shall provide the following information prior to entry for sponsorship:
• Requests for Access Control Lists (ACL) must be submitted/approved NO LATER
THAN SEVEN (7) FULL BUSINESS DAYS before access is required. This allows for adequate time to properly screen personnel and obtain follow-up information if needed. All access control lists requests must include the sponsor’s information. The sponsor must provide the following information on all guests: Last Name, First, MI, (no nicknames) in alphabetical order, Date of Birth and Driver’s License Number with State of Issue. All
ACLs should be submitted via email to 436 SFS/EAL (In the Global Address Book)
(436sfs.eal@us.af.mil).
• ACLs will be for visits under 60 days. The sponsor must be specific on the following.
Reason for Requested Access, Event Location: Provide the building number or the individual’s residence address. All Access Control Lists must be in a Word Document that is attached to the email. Requests submitted in a PDF format or in an Excel document will be mailto:436sfs.eal@us.af.mil returned for correct formatting. Do not paste the Access Control List in the body of the email.
• When requesting access for sub-contractors the following information is needed: full name, Social Security Number, driver’s license number and state of issue, vehicle information (make/model/color/year), license tag number and state, name of company, date and time required for access.
b. Contractor badges will be issued for a maximum period of one year. Prior to re-issuing a badge, all old badges must be returned to the 436 SFS, Pass and Registration office for destruction. Prior to re-issuing a badge, the “Sponsor” must submit a new ORM to the 436
SFS, Pass and Registration office for approval. In the event a badge is lost or stolen, the badge holder must immediately contact the 436 SFS, Pass and Registration office. The badge holder must complete the Loss/Theft of Identification Worksheet and provide to the Contractor’s Site
Supervisor. The Contractor’s Site Supervisor will investigate the loss and report (in writing) the circumstances in which the badge was lost. This information, along with a new ORM, will be submitted by the “Sponsor” to the 436 SFS, Pass and Registration office for re-issuing a badge.
c. When a Contract employee is terminated, the Contractor’s Site Supervisor, COR or RO will immediately notify 436 CONS, who will ensure notification is made to the 436 SFS, Pass and
Registration office. The Contractor’s Site Supervisor, COR, or RO are responsible for returning the contractor’s badge to the 436 SFS, Pass and Registration office. If a terminated employee refuses to surrender the access badge, COR or RO will immediately notify, in the order of precedence: (1) 436 SFS, Base Defense Operation Controller; (2) the 436 SFS, Pass and Registration office; and (3) 436 CONS of the situation. If a base access badge is denied the
436 SFS, Pass and Registration office will notify the “Sponsor” of the denial. The individual in question may contact the 436 SFS, Pass and Registration office for information on how to obtain the reason(s) for denial.
4.3 Installation Access: The contractor shall obtain a personal contractor identification badge for all employees and vehicle passes for all contractor and personal vehicles requiring entry onto
Dover AFB from 436 SFS, Pass and Registration, for the duration of the contract. Employees are only permitted to enter the Installation during the date and time periods indicated on their contractor badge.
a. Vehicle registration, proof of insurance, and a valid driver’s license must be presented for all vehicles while operating on the Installation. All vehicles entering the DAFB Main
Base, Military Family Housing or sensitive areas are subject to search. Any refusal or non-consent by an employee will result in termination of their base access and immediate confiscation of this access badge.
b. During Force Protection Condition (FPCON) Normal and Alpha, personnel without base issued badges must be sponsored onto the installation. During FPCON Bravo, Charlie, and
Delta personnel without base issued identification shall be physically escorted onto the installation.
4.2. Antiterrorism:

b. Terrorism is defined as: The calculated use of unlawful violence or threat of unlawful violence to inculcate fear; intended to coerce or to intimidate governments or societies in the pursuit of goals that are generally political, religious, or ideological.
c. Antiterrorism: As a prelude and during performance of any contract, it is critical that the
Contractor be supportive of all Department of Defense (DoD) requirements to protect personnel and their families, installations, facilities, information, and other resources from terrorist acts. See Attachments 3 & 4 for additional Antiterrorism Information. The following table is a quick reference for the Antiterrorism training:
d. Contractors or subcontractors possessing a Common Access Card (CAC) will complete AT
Awareness Level I training IAW AFI 31-145, Antiterrorism, Standard 25, by completing the―Force Protection computer-based training (CBT) course through the MyLearning site.
This training site is available through the AF Portal at https://lms- jets.cce.af.mil/moodle/. This site automatically records members completion of training and enables the user to print a certificate if needed by the Unit or Base Training Manager.
e. Contractors or subcontractors without a CAC or who do not have access to a government
Non-classified Internet Protocol Router Network (NIPRNET) computer performing construction/services on the installation will complete AT Awareness Level I training IAW AFI
31-145, Antiterrorism, Standard 25, by completing the― Force Protection computer based training (CBT) course through the Joint Knowledge Online (JKO) Learning Management
System (LMS) at http://jko.jten.mil/courses/atl1/launch.html
4.5. Force Protection Condition (FPCON) Impact on Work Levels
a. If the DAFB FPCON is elevated to Charlie, the installation shall evaluate the need for contract services to be discontinued. At FPCON Delta, services are discontinued. Services shall resume when the FPCON level has been lowered.
b. Administrative Note: In the event of an emergency, dial 911 from any land line on the installation. The call will be routed to the ECC (302-677-6666). If a cell phone is used to dial
911, it will be routed to the county 911 dispatch. Inform the dispatcher the emergency is on
DAFB, and they will route the call to the DAFB ECC.

b. Avoid revealing or discussing information/media related to work conducted on the installation to any persons without a valid need to know and adhere to base 100% shred policy when discarding materials.
c. If required, elevated security requirements will be conducted utilizing DD Form 254, Department of Defense Contract Security Classification Specification and appropriately annotated
IAW AFI 16-1406, Air Force Industrial Security Program, for contracts that require additional
OPSEC measures
5. RECORDS MANAGEMENT:
The contractor understands that when creating, handling and maintaining records for the Air Force, either electronic or paper, you must meet the requirements established in AFRIMS Records
Disposition Schedule (RDS), AFI 33-322, Records Management Program. In order to meet the requirement established in accordance with the above-mentioned regulations, contact the Base

6. FLIGHT LINE DRIVING:
IMPORTANT:
a. Vehicle Operations: Individuals that will operate a vehicle on the airfield shall receive driver's training from Airfield Management prior to starting work. Careful attention and strict adherence to the training will prevent accidental damage to aircraft and possible injury to both flight and ground personnel. The training should be completed ASAP but NLT 24 hours prior to the start of the project. Contact 302-677-4183/4185 to schedule a training time.

b. Vehicle operations within the controlled movement area (CMA) are under the control of tower personnel. Control tower radio instructions shall be observed and obeyed by vehicle operators at all times while performing work on the airfield. The Contractor shall assign the Government furnished radio to one of his personnel on the airfield whose responsibility shall be to maintain contact with control tower personnel and coordinate all Contractor movement within the CMA.
The contractor shall check in with Airfield Management each morning, to state the location and type of work to being performed on the airfield and at the end of the day to advise Airfield
Management work is completed for the day. Airfield Management will need to check the construction site for compliance with UFC, ETL and AFI for construction site marking, lighting, obstruction clearance and Foreign Object Damage (FOD) potential. Airfield Barricades:
Barricades shall comply with Unified Facility Criteria (UFC) and Air Force Instructions (AFI).
Barricades shall consist of reflective orange and white low-profile barriers listed in Unified
Facility Criteria Engineering Technical Letters (UFCETLs) or AFIs. Maximum spacing of barricades is 50 feet, with two amber-yellow lights on each barricade. Spacing requirements are found in the UFCs. All barricades shall be either anchored or of sufficient mass to retain an established position where they are placed, in all types of weather conditions. Flashing lights shall be at least 5 candelas effective intensity and flash at a rate of 55 to 75 flashes per minute.
Continuous burning lights must have an effective intensity of 10 candelas.
c. Contractor will be provided a copy of UFC 3-260-01 attachment 14 for Construction Phasing plan and operational safety on airfields during construction.
d. During construction there will be times, due to mission requirements, the contractor will not be authorized to work on site. The times could verify from 24 to 48 hours for work stoppage.
e. Contractor will need to check in and off the airfield daily with Airfield Management.
f. Contractor will provide Dover AFB's Community Planner with equipment, personnel and a map of the construction area for processing of the temporary construction waiver. The construction waiver process can take up to 60 days and the project can not start until the construction waiver is signed.
g. Contractor shall provide Airfield Management with the equipment list (type, color of vehicle and license plate), personnel (full name and SSN) for processing of the controlled area exception work paperwork. This process could take up to 45 days for approval.
7. GENERAL INFORMATION.
Hours of Operation. The base hours of operation are 7:00AM to 4:30PM, Monday through
Friday. The Contractor is not required to provide service except for emergencies on the following days: New Year's Day, Martin Luther King's Birthday, Washington's Birthday, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.

Antiterrorism:
As a prelude and during performance of any contract, it is critical that the contractor be supportive of all Department of Defense (DoD) requirements to protect personnel and their families, installations, facilities, information, and other resources from terrorist acts.
All contractors and subcontractors are responsible for providing their personnel information on Dover Air Force Base (DAFB) Antiterrorism (AT) Program. During performance of work, personnel security and protection of resources is critical for the installation to maintain a safe work and secure environment.
Contractors must ensure that contracted personnel and vehicles are strictly controlled during performance of duty. For instance, contractors shall use reasonable efforts to prevent any personnel or equipment from being placed in harm’s way or in a position that could aid or abet terrorists (unattended running vehicle).
Antiterrorism Awareness Level I Training:
DoD Contractors or subcontractors possessing a Common Access Card (CAC) can complete AT Awareness Level I training IAW DAFI 31-145, Antiterrorism, Standard 25, by completing the―Force Protection computer based training (CBT) course through the MyLearning site. Emergency 9-1-1:
When dialing 911 from your cell phone, you need to tell the dispatcher your location i.e. Dover AFB and they will transfer you to DAFB Emergency Communications Center (ECC).

What to report:
Surveillance – Using cameras (either still or video), note taking, drawing diagrams, writing on maps, or using binoculars or other vision-enhancing devices to record or monitor base activity
Elicitation – Attempts to gain information from you by mail, in person, or by telephone about military operations, capabilities, or people
Tests Of Security - Attempts to measure reaction times or penetrate physical security barriers or procedures in order to assess strengths and weaknesses
Acquiring Supplies – Purchase, theft, or unauthorized acquisition of explosives, weapons, ammunition, military uniforms, decals, flight manuals, passes or badges, or any other controlled items
Suspicious Persons Out Of Place – People who don’t seem to belong in the workplace, neighborhood, business establishment, or anywhere else
Dry Run – People getting into position and moving around according to their plan without actually committing a terrorist act. This could also include mapping out routes and determining the timing of traffic lights and flow
Deploying Assets – People and supplies getting into position to commit the act. This is a person’s last chance to alert authorities before the terrorist act occurs
Funding – Suspicious transactions involving large cash payments, deposits, or withdrawals are common signs of terrorist funding. Collections for donations, the solicitation for money and criminal activity are also warning signs

Victim By Association – The wrong friends, in the wrong place, at the wrong time Victim By Location – The wrong place at the wrong time Victim By Opportunity – The completely random circumstance, primarily occurs at a soft target
What To Do If You Spot Suspicious Terrorist Activity If you see suspicious behavior, do not confront the individuals involved.
Take note of the details:
S – Size (Jot down the number of people, gender, ages, and physical descriptions) A – Activity (Describe exactly what they are doing) L – Location (Provide exact location) U – Uniform (Describe what they are wearing, including shoes) T – Time (Provide date, time, and duration of activity) E – Equipment (Describe vehicle, make, color etc., license plate, camera, guns, etc.)
Be Prepared To:
Give your name and company
Give your location (as clear as you can, give a building name, number or street) Provide a call back phone number If safe, remain at your location until contacted by Security Forces Move to a location as directed by the ECC

INFORMATION COUNTERMEASURES
Security Measures Report suspicious behavior immediately Personal Identifiable
Information (PII) (Privacy Act) Use personal info only when required, utilize Privacy Act coversheets, adhere to 100% shred policy, encrypt when emailing, password protect documents when encryption is not available. Do not display info in public areas and verify need to know. Personal Info includes, but is not limited to:
SSANs, home addresses, and family members.
Mission Capabilities Restrict access to sensitive information such as aircraft, personnel, logistics, and supplies. Protect resources, follow a strict shredding policy, ensure sensitive facts are secure, and limit access to status boards.
Force Readiness Ensure secure communication by using email encryption when discussing information such as role of personnel, equipment, and manning strength reports. Do so only in authorized settings with authorized personnel. Use secure communication channels, controlling access to information, and limiting access to those who need to know.
Force Structure Avoid posting information relating to force structure procedures or email correspondence in public areas, follow a strict shredding policy, encrypt emails, and password protect documents when encryption is not possible to ensure sensitive information remains secure.
Exercises and Plans Reveal only technical details of mission related materials to those that need to know such as Project Manager, Inspector, Contracting Officer Representative, Contracting Specialist, and Contracting Officer. Do not post on social media any information relating to the contract or any events or situations occurring in government's premises. Adhere to
100% shred policy.
Physical Infrastructure Only disclose information such as diagrams, detailed maps, or any type of imagery, among others, to authorized personnel. Transmit information over approved channels, avoid discussing in open environments, and limit access to those who need to know.
