Performance Work Statement for
355 CMS/MXMCE Receptacle and Installation Services
1.1. Description of Services. The contractor shall furnish all labor, parts, tools, materials, equipment, transportation, personnel, management, and supervision necessary to install two 60 Amp receptacles in 355 CMS Bldg. 5045 on Davis-Monthan Air Force Base (DMAFB), AZ, in accordance with manufacturer’s specifications, industry standards, and business practices.
1.2. Removal of Defective Hardware. The contractor shall remove the damaged and improper connectors. The contractor shall remove all required electrical components and hardware required to properly replace the unit. This includes, but is not limited to, wiring, bolts, nuts, terminal lugs, etc. The contractor should make every effort to prevent damage to the surrounding facilities and shall restore any affected areas to their original condition.
1.3. Receptacle Specifications and Installation. The contractor shall furnish and install two
115/230 VAC, 4-wire, 3-phase, 60-amp receptacles in Building 5045, 355 CMS, DMAFB, AZ, in accordance with the Salient Characteristics provided. Each receptacle shall be wall-mounted and connected to the existing General Duty Safety Cut-Off Switch when feasible. The receptacles shall be compatible with Purge Unit Plug, Cat No. CPH-6034BC.
The installation shall not interfere with existing equipment or operations. The contractor shall provide all components necessary for a complete and functional installation, including wiring, caps, mounting hardware, and terminal lugs. Receptacles shall be rated for Hazardous Location Class I, Group C, and shall include delayed-action contacts to ensure safe interconnection. Upon completion, the contractor shall perform functional testing to verify proper operation and shall ensure the system is fully operational before departing the site.
1.3.1. Shop Drawings. The contractor shall submit a drawing or specification sheet for the receptacle to ensure compatibility with existing plug.
1.3.2. Schedule. Upon contract award, and before beginning work, the contractor shall provide the Contracting Officer and the 355 CMS point of contact (520-228-3702) with an estimated timeline for completing the required services. The contractor shall provide updates if the timeline changes. This project should be completed within 7 days after receiving all materials/parts to complete the job.
1.3.3. Working Hours. The contractor shall perform installation services between the hours of
7:30 a.m. and 3:30 p.m., Monday through Friday, excluding Federal Holidays. The contractor shall coordinate with the 355 CMS point of contact (520-228-3702) to schedule specific days and times to perform services. If the contractor desires to work after 3:30 p.m., weekends, or during Federal Holidays, the contractor must submit the request to the Contracting Officer. Each request will be considered on a case-by-case basis.
1.3.3.1. Restricted Areas/Government Escort. Building 5045 is not a restricted area.
1.4. General Information
1.4.1. Contract Manager. The contractor shall identify a primary point of contact who will be responsible for coordinating and communicating all contractor-performed services. This individual shall remain reachable by phone whenever contractor or subcontractor personnel are on site. Upon contract award, and before work begins, the contractor shall provide the name and telephone number of this point of contact, in writing, to the Contracting Officer and the 355 CMS point of contact. The designated individual shall have sufficient authority to address day-to-day matters related to performance of the contract.
1.4.2. Contractor Qualifications. All contractor’s employees shall be certified and qualified to perform equipment installation. The contractor shall provide proof of employee certification or qualification within one business day upon the Government’s request. All work shall be accomplished in accordance with the National Electric Code, ANSI/NFPA 70, and all other applicable laws and standards. The contractor is responsible for all code compliance and proper installation.
1.4.3. English Language. All contractor employees must be able to read, write, speak, and understand English fluently.
1.4.4. Utility Outages. If any building utilities must be shut down to complete the installation, the contractor shall notify the Contracting Officer in writing at least 21 days prior to the requested outage date. The contractor is responsible for all work associated with any required utility outages.
1.4.5. Contracting Officer Authority. The Contracting Officer is the only Government official that can make any changes to the contract. If the contractor is asked to make changes or deviate from the contract, the contractor shall not make the changes and notify the Contracting Officer immediately.
1.4.6. Contractor Travel on DMAFB. Contractor employees shall not loiter in any working area before or after performing services. All contractor employees shall limit their travel on DMAFB only to the specific areas required for performance of services or in travel directly to and from these locations. Upon completion of contract work, contractor employees shall promptly depart DMAFB.
1.4.7. Contractor Employee Conduct. The Government reserves the right to require removal from the installation any contractor employee who endangers persons or property, whose actions are inconsistent with professional conduct, whose continued employment is inconsistent with the interest of military security or who is found to be incapacitated or under the influence of alcohol, drugs, or other substances. Removal of employees for any reason does not relieve the contractor of the requirement to perform services specified herein.
1.4.8. Appearance. Employees shall present a neat appearance and be easily recognized as contractor employees by company uniforms, or name tags having the company logo affixed. All contractor employees must have in their possession at all times; Company ID badges and any Government Base access passes issued to them.
1.4.9. Safety. The contractor shall comply with all local, state and federal laws and safety regulations. Compliance with OSHA and other applicable laws and regulations for the protection of contractor employees is exclusively the obligation of the contractor, and the Government will assume no liability or responsibility for the contractor’s compliance or noncompliance with such responsibilities. The contractor is responsible for furnishing contractor employees with all necessary personal protective equipment to perform services.
1.4.9.1. Work Areas. The contractor shall, at all times, keep work areas free from accumulations of waste material, rubbish, tools, equipment, and materials. Upon completion of the job or when away from the work site, the contractor shall leave the work area and premises in a clean, neat, safe and workmanlike condition. No asbestos-containing materials may be used or installed in this work.
1.4.9.2. Trash/Waste Removal. The contractor shall remove and dispose of all trash and waste materials at a permitted site. The contractor shall not dispose of any trash or waste on
DMAFB.
1.4.9.3. Smoking in AF Facilities. Contractors are advised that the AF has placed restrictions on the smoking of tobacco products in AF facilities. AFI 40-102, Tobacco Use in the Air Force, outlines the procedures used by the commander to control smoking in our facilities. Contractor employees and visitors are subject to the same restrictions as Government personnel. Smoking is permitted only in designated smoking areas.
1.4.9.4. Illegal Substances. Illegal substance possession and/or use will not be tolerated on any military installation, including DMAFB. This includes the use, possession, distribution, or purchasing of marijuana, even if it has been medically prescribed. The contractor shall ensure that contractor employees who have base access do not possess and/or use illegal substances. All contractors and subcontractors are required to adhere to all local, state and federal laws concerning possession and/or use of illegal substances. Contractors found to be in possession of illegal substances will be barred from the installation, and subject to arrest and prosecution by local authorities.
1.4.10. Federal Holidays. The contractor is not required to provide services on the following

1.4.11. Security Requirements
1.4.11.1. List of Employees. Upon contract award, the contractor shall submit to the
Contracting Officer a list of contractor employee names, social security numbers, and driver’s license numbers, or other means of positive identification of all employees performing services on DMAFB. Foremen, supervisors, and superintendents must be identified on the list of employees. The contractor shall submit an updated list when an employee's status or information changes. The name of any contractor employee that is terminated during the period of the contract must be reported to the CO in writing within one workday of the employee’s termination.
1.4.11.2. Safeguarding of Classified and Other Information. The contractor shall ensure that contractor employees do not notify, repeat, comment upon, disclose, or use in any way information seen, observed, or derived from documents or conversations of a sensitive or confidential nature overheard by them. The contractor shall comply with, and assume responsibility for compliance of this provision by its employees.
1.4.11.3. Pass and Identification Items. The contractor shall ensure the pass and identification items required for contract performance on DMAFB are obtained for employees and non-Government owned vehicles.
1.4.11.4. Retrieving Identification Media. The contractor shall retrieve all identification media, including vehicle passes from employees who depart for any reason before the contract expires; e.g. terminated for cause, retirement, etc, or upon contract completion.
The contractor shall notify the Contracting Officer in writing when an employee departs, and return the employee’s identification to the office that issued it.
1.4.11.5. Traffic Laws. Contractor employees shall comply with all Davis-Monthan AFB traffic regulations and state traffic laws. Contractor employees are subject to random vehicle speed control checks. Failure to adhere to base traffic regulations may result in loss of base driving privileges, debarment from the installation, or other administrative action. The use of cell phones is prohibited while driving on Davis-Monthan AFB unless the device is hands-free, and seat belt use is mandatory for all drivers and passengers. All contractor personnel operating a vehicle on the installation must possess a valid driver’s license that meets REAL ID requirements as outlined in Section 1.5.3 of this document.
1.4.11.6. Random Personnel and Vehicle Searches. Contractor personnel are subject to random personnel and vehicle searches. If contractor personnel refuse to be searched, they will be denied entry to the base. Denial of entry to contractor personnel who refuse to be searched, or detention of personnel found to be in possession of contraband, may result in loss of base driving privileges, debarment from the base, or other administrative action. Detention of contractor employees for any reason does not relieve the contractor of the requirement to perform contract services.
1.4.11.7. Weapons, Firearms, and Ammunition. Contractor employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within their contractor owned vehicle or privately owned vehicle while on Davis-Monthan AFB.
1.4.11.8. Controlled Unclassified Information (CUI). The contractor shall comply with
DoDI 5200.48, Controlled Unclassified Information (CUI), and all applicable requirements for marking, handling, transmitting, and safeguarding CUI. Any CUI or other sensitive information encountered by contractor personnel shall not be retained and must be immediately surrendered to an appropriate DMAFB authority.
1.4.11.9. Reporting Requirements. Contractor employees shall immediately report to an appropriate DMAFB authority any information or circumstances of which they are aware that may pose a threat to the security of Department of Defense personnel, contractor personnel, resources, and classified or unclassified defense information.
1.4.11.10. Physical Security. Contractor employees are responsible for safeguarding all
Government property and controlled forms provided for the contractor’s use. At the end of each work period, the contractor shall secure all DMAFB facilities, equipment, and materials.
1.4.11.11. Base Entry Requirements. The contractor shall obtain Government identification documents for all employees by applying through the 355 CMS point of contact. The contractor must provide each employee’s full name, Social Security number, driver’s license number (including state), and required hours of access.
Employees without a driver’s license must present another valid form of government-issued photo identification. Any employee requesting to bring a privately owned vehicle onto Davis-Monthan AFB must provide proof of vehicle registration, minimum required liability insurance, and a valid driver’s license that meets REAL ID requirements as outlined in Section 1.5.3 of this document. Contractor ID cards or temporary base passes will be available for pickup within seven working days after the application is received.
1.4.11.12. Contractor Vehicle Requirements. All vehicles used in the performance of the contract shall be properly registered and meet local, state, and federal safety and emissions requirements. Each contractor vehicle shall have the Contractor Logo neatly exhibited on each side of the cab. The sign must use at least 2" high letters. Vehicles shall be in satisfactory mechanical condition and present a neat and clean appearance.
Any vehicle(s) not so maintained may be ordered removed from the base by and at the sole discretion of the CO, until satisfactory repairs are made. The contractor shall operate vehicles to comply with base traffic regulations. Vehicle-mounted communication equipment shall conform to all applicable base and federal frequency regulations and avoid conflict with existing frequencies.
1.5. GOVERNMENT FURNISHED PROPERTY AND SERVICES
1.5.1. Utilities. A source of electricity will be available for the contractor’s use.
1.5.2. Emergency Services. The Government will provide security forces services (call 228-
4444, for emergencies only), and fire protection services (call 228-3333). There are no emergency medical services available on Davis-Monthan AFB, therefore the contractor must dial 911 for emergency medical services.
1.5.3. Real ID Requirement. Beginning May 7, 2025, Davis-Monthan Air Force Base will require all visitors seeking base access to possess a REAL ID in accordance with the REAL ID Act of 2005 and DoD Manual 5200.08 Vol 3, Physical Security Program:

Federal installations, including Davis-Monthan AFB, will no longer accept non-marked state-issued identification cards after May 6, 2025. As mandated by the Department of Homeland Security (DHS), state-issued licenses and identification cards must include a REAL ID-compliant marking–star markings, “Enhanced” verbiage, etc.—indicating adherence to federal standards. Enhanced Driver’s Licenses (EDLs) issued by Washington, Michigan, Minnesota, New York and Vermont are also acceptable for federal purposes. Although most EDLs do not contain the REAL ID star marking, they meet DHS criteria for access.
To obtain a REAL ID, individuals must visit their state’s driver licensing agency website for documentation requirements. At a minimum, applicants must provide proof of full legal name, date of birth, Social Security number, two proofs of address and lawful status. States may impose additional requirements. Minors under 18 years of age are not required to present REAL ID-compliant credentials for travel or base access, but their accompanying adult must be compliant.
Individuals issued a “LIMITED-TERM” REAL ID—commonly non-citizens with documentation such as a Permanent Resident Card, visa, or employment authorization— must present the same documentation when requesting access to DMAFB.
Common REAL ID-compliant credentials include U.S. passports, permanent resident cards and other identification approved under DODM5200.08V3. Current Defense Biometric Identification System (DBIDS) badges are not impacted by this change.
However, new or replacement DBIDS badges will only be issued upon presentation of REAL ID-compliant credentials. When screening visitors for DBIDS Contractor Badges and EAL sponsorship visitation requests, it is the direct responsibility of the DoD-affiliated sponsor to verify that the personnel they are requesting badging or visitation for are REAL ID compliant.
For DBIDS contractors, Visitor Control Center badging personnel will confirm contractors’ REAL ID compliance when they come to receive their DBIDS badge.
Security Forces personnel will verify visitors’ driver’s licenses and identification cards to ensure REAL ID compliance is met. Non-compliance will result in immediate denial to the installation until REAL ID compliance is met.
Security Forces entry controllers will verify REAL ID compliance of visitors when verifying their credentials with Entry Authorization Lists. Non-compliance will result in immediate denial to the installation until REAL ID compliance is met.
