About this file
Exhibit A is a Vendor Disqualification (DQ) Checklist for the Colorado Department of Corrections (CDOC) Warehouse Fresh Produce Order. The document outlines the mandatory submission requirements for vendors bidding on this weekly produce contract, which involves delivering fresh produce to 6 warehouse locations. Vendors must submit their quotes through bidnetdirect.com and include specific required exhibits: Exhibit B (Vendor Information), Exhibit C (W-9), Exhibit D (Price Proposal), and Exhibit E (Vendor Disclosure Statement).
The checklist emphasizes precise submission guidelines, requiring vendors to state the DQ number and title on their quotes, and sign or electronically validate the Required Vendor Information form. All documents must be completed and returned as part of the bid/quote submission package. The checklist serves as a guide to ensure vendors provide all necessary documentation for consideration in the CDOC Warehouse fresh produce procurement process, with an explicit instruction for vendors to review all remaining exhibits and attachments associated with the bid opportunity.
