About this file
Exhibit A is a Vendor Disqualification (DQ) Checklist for the Colorado Department of Corrections (CDOC), Division of Colorado Correctional Industries (CCi) Canteen Services solicitation for Watches & Glasses. The document outlines the required submission documents for the bid/quote process, which must be delivered to bidnetdirect.com and include: Exhibit B (Required Vendor Information) signed in ink or electronically, Exhibit C (W-9 Request for Taxpayer ID Number) completed and returned, Exhibit D (Price Proposal), and Exhibit E (Vendor Disclosure Statement).
The checklist serves as a guide for potential vendors to ensure they have included all necessary documentation for the Watches & Glasses procurement for the Canteen (Commissary) program. The document emphasizes the importance of carefully reviewing all exhibits and attachments, and submitting a complete bid package that clearly states the DQ number and title. The solicitation supports the Canteen Services' mission of providing employment opportunities for offenders and offering a varied selection of goods at competitive pricing within the Colorado correctional system.
