About this file

This is a Vendor Disqualification Checklist document outlining submission requirements for a state or local contract opportunity. The checklist specifies that all quotes must reference the DQ number and title, and all submissions must be delivered through bidnetdirect.com. Four exhibits are required with bid submissions: Exhibit B containing required vendor information with signature in ink or electronic format, Exhibit C containing a completed W-9 Request for Taxpayer ID Number, Exhibit D containing the price proposal, and Exhibit E containing the vendor disclosure statement. Bidders are also required to review all remaining exhibits and attachments included with the solicitation documentation.

The checklist serves as a pre-submission verification tool to ensure vendors have prepared and included all mandatory documentation before final submission. No specific information regarding pricing terms, set-asides, award dates, contract term, or renewal options is provided in this document, as it functions solely as a compliance verification checklist for the bidding process.
