About this file

This document is an Addendum to FAR Clause providing quote preparation instructions for a cafeteria services contract (NIEHS21900003) at the National Institute of Environmental Health Sciences (NIEHS). The solicitation requires vendors to submit a quotation consisting of three separate volumes: Technical Capability (20-page limit), Past Performance (3 pages per reference), and Price, with specific formatting requirements including MS Word or PDF formats.

Vendors must provide a comprehensive technical approach including a 15-day transition plan, a detailed quality control plan covering service delivery and contingencies, and a staffing plan demonstrating personnel qualifications. The past performance volume requires a minimum of three references from the past three years, highlighting service quality, timeliness, and customer satisfaction. The price volume must include a cover letter, point of contact information, representations and certifications, and a pricing table with monthly and yearly service prices rounded to whole numbers. The solicitation emphasizes detailed, clear documentation of the vendor's capability to operate an onsite cafeteria without interruption.
