About this file

This document is a Bid Submission Form for a municipal contract with a city government, designed for potential contractors to submit their bid and certify their compliance with various municipal requirements. The form requires bidders to provide comprehensive company details, certify non-discrimination practices, and agree to maintain documentation demonstrating fair subcontractor solicitation. The bid is valid for 120 calendar days from the bid due date, and the city reserves the right to reject any or all bids, make awards on all or specific items, and waive formalities.

The form emphasizes strict anti-discrimination policies, requiring bidders to disclose any discrimination complaints filed against them in the past ten years and certify that they have not engaged in prohibited discrimination in subcontractor selection. Additionally, bidders must confirm that none of their owners, employees, directors, or contractors will violate the city's Conflict of Interest Policy. The document serves as both a submission form and a legal certification, compelling bidders to provide accurate, complete information and acknowledging potential contract termination for false statements or failure to provide requested documentation.
