PROJECT DETAILS
State the purpose of the project and provide project details for the length of the budget period (11 months).
For the selected program strategies:
1. Describe proposed activities and provide sample process measures to track progress implementing the activities
2. Describe the expected outputs (resources, tools, products, etc.) and outcomes (change to occur), and provide sample outcome measures to track progress achieving the outcomes.
Project Purpose
On any given night in 2019, approximately 568,000 people in the United States experienced homelessness.
Water, sanitation, and hygiene (WASH)-related health emergencies (emergencies caused by WASH diseases) are public health problem among people experiencing homelessness (PEH). Outbreaks of Shigella, hepatitis A, and group A streptococcal skin infections among PEH have recently been reported in multiple US jurisdictions.
Inclusion of PEH is often missing from WASH-related public health emergency plans. To date, little work has been done to systematically assess local health department needs related to response planning for WASH-related public health emergencies among sheltered and unsheltered PEH. Additionally, few resources are https://cdcpartners-368deb5c5533c2.sharepoint.com/sites/CSTLTS/COS/COSP/FormsApp/UFRuntime.aspx?remoteAppUrl=https://formso365.nintex.… 4/9 available for local jurisdictions to initiate response planning for WASH-related health emergencies among sheltered and unsheltered PEH.
This project will assess public health jurisdiction needs related to WASH-related public health emergencies among sheltered and unsheltered PEH, and support the development and evaluation of preparedness plans in select jurisdictions.
147/150 words maximum
Program Strategy: Leadership and Workforce Development
Activities *
Support 3-6 local health department jurisdictions (from georgraphically diverse regions) with large populations of PEH to develop and implement preparedness plans for WASH-related health emergencies. Response plan should be focused on both sheltered and unsheltered populations and should focus on public health emergencies (defined as emergencies caused by WASH related pathogens such as Shigella, Hepatitis A, and group A streptococcus). Preparedness plans should also include information about different WASH-related diseases.
Conduct a process evaluation among the 3-6 jurisdictions developing response plans to describe the process of developing and implementing the plan.
Using information from the needs assessment (see Activity in Program Strategy: Programs and
Services ) and the process evaluation create a toolkit that other jurisdictions can use to develop preparedness plans for WASH-related health emergencies among sheltered and unsheltered PEH.
Toolkit should include templates, guidance, example materials, information about implementation experiences from funded jurisdictions, lessons learned and stories from the field, strategies to form partnerships and overcome barriers, and example strategies that have worked in funded jurisdictions.
Toolkits should include information specific to both sheltered and unsheltered PEH and about different
WASH-realted diseases.
Disseminate preparedness plans developed by 3-6 funded jurisdictions to a range of stakeholders, including unfunded local health department jurisdictions. Use a range of strategies including virtual symposia, conference presentations and roundtables, and virtual workshops to share preparedness plans with other jurisdictions.
Disseminate the toolkit developed to support non-funded jurisdictions through to a range of stakeholders, including to non-funded jurisdictions and health departments. Use a range of strategies including virtual symposia, conference presentations and roundtables, and virtual workshops to share preparedness plans with non-funded jurisdictions.
Evaluate the intention and capacity to create preparedness plans for WASH-related emergencies among sheltered and unsheltered PEH among non-funded jurisdictions following dissemination https://cdcpartners-368deb5c5533c2.sharepoint.com/sites/CSTLTS/COS/COSP/FormsApp/UFRuntime.aspx?remoteAppUrl=https://formso365.nintex.… 5/9 activities.
Conduct an outcome evaluation among the 3-6 funded jurisdictions to assess impact of developing and implementing the WASH-related preparedness plans for WASH-related emergencies among sheltered and unsheltered.
Process Measures *
Number of jurisdictions supported to develop preparedness plans
Number of jurisdictions supported to implement preparedness plans
Number of process evaluations completed
Number of dissemination activities to share preparedness plans (e.g., webinars, meetings)
Number of dissemination activities to share toolkit (e.g., webinars, meetings)

Outputs *
Toolkit for developing preparedness plans for WASH-related health emergencies among PEH
Process and Outcome evaluation summary report
Dissemination activity agenda, slide-decks, presentations
Jurisdiction preparedness plan
Budget Period Outcomes *
Local health departments will have improved capacity to develop preparedness plans for WASH-related public health emergencies, and to customize these plans to their local jurisdiction needs.
Local health departments will have increased capacity to implement preparedness plans for WASH-related public health emergencies which will serve to address public health needs of sheltered and unsheltered people experiencing homelessness in their jurisdiction.

PEH data
Jurisdictions that participated in creating preparedness plans report establishing connections with PEH organizations and partners
Jurisdictions that participated in dissemination activities report intention to create response plans

Program Strategy: Programs and Services
Activities *
Conduct a needs assessment with local health department partners to examine needs related to
WASH-related health emergencies among sheltered and unsheltered PEH. Needs assessments should have questions focused on differences in needs related to sheltered and unsheltered PEH, and focused on different WASH-realted diseases. All local health departments will be invited to participate in the needs assessment. Needs assessment should include either qualitative, quantitative, or both methods of data collection. Needs assessment should explore local needs, strategies used in the past to address WASH-related health emergencies among PEH, existing partnerships with organizations supporting PEH, challenges responding to WASH-related health emergencies among PEH, and challenges developing response plans for WASH-related health emergencies among PEH. Needs assessment should include stakeholders from a range of jurisdictions in the United States and stakeholders at multiple levels in public health organizations.

To build capacity of state health departments to communicate the impact of their funded injury and violence prevention (IVP) programs. Begining in FY2020, CDC received historic funding for three new programs:
adverse childhood experiences (ACEs), suicide prevention, and firearm injury and mortality prevention.
Leveraging this funding, CDC funded health departments to respond to these public health issues. Health departments and their partners must be equipped to share compelling stories around these programs to garner interest, encourage partnership, and demonstrate need for continued support. This project will gather health departments and other relevant stakeholders for interactive conversations to better understand and tell the stories of hope and success from these newly funded programs. The project will build the capacity og these health departments to develop and deply storytelling communication products from the most promising stories using various mediums (potentially presentation graphics, one-pagers, quotes, audio clips, or narrative pieces).
