About this file

This document comprises solicitation response forms required by the Tohopekaliga Water Authority (Toho) for vendors responding to formal solicitations. The forms establish standardized requirements and certifications that all vendors must complete and submit with their bids or proposals. The solicitation relates to the Lift Station - 05C (Toho East Lakeshore) Rehabilitation Project in Florida, which involves the repair or complete replacement of an existing quadplex master lift station currently handling an estimated 2.2 million gallons per day average flow with peaks of 2.9 million gallons per day. The project aims to improve pump station efficiency and reliability while bringing the facility to current operational standards. Vendors must acknowledge all addenda issued during the solicitation process and certify their understanding of project requirements through notarized signature pages.

Vendors responding to Toho solicitations must provide multiple compliance certifications covering federal and state legal requirements, including E-Verify registration, public entity crimes compliance, non-collusion affidavits, federal debarment and suspension status, drug-free workplace certification, and anti-human trafficking attestations. Additional requirements include certifications regarding vendor eligibility under Florida statutes covering discriminatory practices, antitrust violations, Israel boycott prohibitions, foreign country of concern restrictions, common carrier status, and trench safety compliance. Vendors must also submit proof of similar project experience and references on forms TOH-130, maintain acknowledgment of confidential structural records, and provide documentation confirming previous submission of required standard forms for prequalified contractors. All certifications require execution by authorized representatives with notarization, with penalties of perjury applied to false statements. The forms versions range from revision dates between February 2025 and March 2024.
