The form is associated with a Ground Storage Tank Inspection, Maintenance and Repair Services contract and must be completed and submitted either with the initial vendor submittal or within three business days of the County's request, with potential consequences of being deemed nonresponsive for non-compliance.

The related state and local contract opportunity is an open-end contract for Ground Storage Tank Inspection, Maintenance and Repair Services for Water & Wastewater Services and other Broward County agencies. Vendors must submit their solicitation responses electronically through BPRO, ensuring they receive a Submission Receipt. The solicitation requires vendors to adhere to county/state licensing requirements and provides a specified timeline for submitting questions and clarification inquiries through the Bonfire BPRO platform. Technical support is available through support@gobonfire.com if vendors experience difficulties with the submission process.
