About this file
This document is an Affirmation and Disclosure Form from Crown Consulting, Inc., specifically related to a state contract with the Ohio Department of Transportation (ODOT). The form addresses compliance with Executive Order 2022-02D, which prohibits state agencies from purchasing services from Russian companies or institutions. Crown Consulting, located at 1530 Wilson Blvd., Suite 900 in Arlington, VA 22209, affirms that all services will be performed within the United States and that no state data will be located or accessed outside the United States without a signed waiver.
The form does not specify detailed contract specifics such as pricing, goods, or services, but it does emphasize the contractor's commitment to performing all work domestically and providing transparency about service locations. The document requires the contractor to disclose any changes in service locations before, during, and after contract execution, with ODOT retaining the right to immediately terminate the contract if services are shifted outside the United States. The form was signed by Tammy Shippy, Contracts Manager, on May 14, 2025, and appears to be part of a broader contractual agreement, potentially related to uncrewed aircraft systems, docks, and parts as suggested by the government file name.
