About this file

This is an Exception Form document for KIPP Texas Public Schools' E-Rate Category 2 Equipment and Installation Request for Proposal. The form serves as the mechanism through which proposers must formally declare any deviations or exceptions to the RFP specifications, terms, conditions, requirements, and obligations. The RFP itself seeks qualified vendors to provide lifecycle replacements and installation of equipment across five sites located in the Dallas-Fort Worth, Houston, and San Antonio regions within Texas. The scope of work requires a turnkey proposal encompassing replacement, installation, configuration, testing, labeling, and inventory of specified components. All cabling costs must include removal of existing cable infrastructure, terminations, enclosures, bulkheads, patch cables, and conduit/trenching/boring work as necessary to complete the installation.

The Exception Form requires that proposers quote exact RFP language to which they take exception and specify their particular concerns with that language. The form mandates signature by each proposer regardless of whether exceptions are listed, and it must be submitted with the proposal. Proposers must identify their company name and include the name, title, and signature of a corporate officer authorized to execute the proposal. The form indicates that additional pages may be appended if necessary to accommodate multiple exceptions.

EXCEPTION FORM

All deviations and exceptions to this RFP must be expressly stated in this Exception Form (additional pages to this form may be added if necessary). In the absence of any entry on this Exception Form, the Proposer(s) assures KIPP of their full agreement and compliance with all specifications, terms and conditions, requirements, and obligations of the RFP. THIS EXCEPTION FORM MUST BE SIGNED BY EACH PROPOSER(S) WHETHER THERE ARE EXCEPTIONS LISTED OR NOT AND SUBMITTED WITH THE PROPOSAL. Proposers should respond to this section by quoting the exact language in the RFP that they take exception to and then indicate what the specific concern is with the language.
