About this file

This document is Appendix B: Required Forms for Request for Quotations #27-02 issued by Monterey-Salinas Transit District (MST) in California for Actuarial Services. The RFQ solicits quotations from qualified consultants to provide actuarial services for MST's self-insured workers' compensation and liability programs. Quotations must be submitted through MST's Bonfire eProcurement Portal by Wednesday, December 2, 2026, at 5:00 p.m. PST. The selected consultant will enter into a contract effective January 1, 2027, for an initial term of two years through December 31, 2028, with MST holding the option to extend the contract for two additional two-year terms.

The appendix contains six required forms that proposers must complete and submit with their quotation: a Certification of Contractor Regarding Debarment, Suspension, and Other Responsibility Matters; a Federal Tax Delinquency and Conviction Certification; a Non-Collusion Affidavit; a Proposer's Statement Regarding Insurance Coverage; an RFQ Compliance or Exception declaration; and a Cost Proposal Form. The Cost Proposal requires proposers to submit a Grand Total Price Per Actuarial Study as a fixed price representing all costs for materials, equipment, delivery, warranties, insurance, services, and other necessary costs for the contract's duration. The insurance requirements specify that the consultant must maintain Professional Liability insurance of $2,000,000 per claim and in the aggregate, Commercial General Liability of $1,000,000 per occurrence and $2,000,000 general aggregate, Commercial Automobile Liability of $1,000,000, and Workers' Compensation insurance of $1,000,000, with all policies naming MST as an additional insured.
