About this file
This is a Non-Collusion Affidavit required by the City of Lubbock, Texas for the Phase 7 Waterline Change Out project. The affidavit serves as a sworn statement in which bidders certify that their submitted bid is genuine and not collusive in nature. Bidders must affirm that they have not directly induced or solicited any other firm to submit a sham bid, refrain from bidding, or otherwise collude to gain competitive advantage. The form requires completion with the bidder's firm name, authorized representative's name and signature, title, and the date of execution. A notary public must witness and authenticate the affidavit, with the notary's commission expiration date included.
This document is a mandatory submission requirement for all bids on the Phase 7 Waterline Change Out project and must be included with each proposal submission. No pricing information, set-asides, or other financial terms are specified within this affidavit form itself, as it functions solely as a compliance and integrity certification document required by the City of Lubbock.
