CONTRACTOR’S RESPONSIBILITY CERTIFICATION FORM
Each prime bidder shall complete the Contractor’s Responsibility Certification Form provided, and shall submit the Certification Form to the Township, to the attention of Patricia Platt, via E-mail to rco@abingtonpa.gov, no later than fourteen (14) days prior to the bid opening date, together with all documentation required by Section 3 (c) of Ordinance 2193, a copy of which is provided, in order to be pre-qualified before submitting a bid.
Specifically, each Certification Form shall be accompanied by documentation providing a list of all trades and classifications of craft employees it will employ on the project and the names and addresses of all applicable apprenticeship programs relating to craft personnel utilized on the project and verifying that the bidder participates in a Class A Apprenticeship Program for each trade or classification listed.
These requirements also apply to all proposed subcontractors. Accordingly, prime contract bidders, once pre-qualified and identified as the likely low bidder, are required to identify all subcontractors in order to obtain the subcontractors’ certifications and supporting documentation, as outlined herein and in the Ordinance. Upon issuance of a Notice of Intent to Award the contract, the proposed successful bidder will have only fourteen (14) days to submit the required documentation for subcontractors; failure to meet that deadline will result in the bidder being disqualified.
The Township will notify the prime contract bidder within seven (7) days of receipt of the pre-qualification submission, of the determination of the bidder’s pre-qualification to submit a bid. Failure of the prime contractor to submit the required Certification Form and supporting documentation in a timely manner, as required for pre-qualification, will automatically disqualify any firm from submitting a bid.
