Requirements for National Health Interview Survey (NHIS) Centralized Trainings September 4-8, 2018

· Pre-function area for registration

· Room to store materials for training (pallets)

· Sleeping Rooms

· Tuesday, September 4 – 300

· Wednesday, September 5 – 525

· Thursday, September 6 - 525

· Friday, September 7 – 300

· Saturday, September 8 - 22 Meeting Space (7:30 A.M. – 5:00 P.M) 24 hour hold on all space, including breakouts

· One Ballroom (ballroom #1) needed on Wednesday, September 5, seating 300 classroom style (Details below) for session #1

· One Ballroom (ballroom #1) needed on Thursday, September 6, seating 300 classroom style (Details below) for session #1

Total of 3 ballrooms large enough to hold about 300 people and 3 breakout rooms large enough to hold up to 125 people classroom style.

Cities Solicited are:

Boston, Massachusetts Denver, Colorado National Harbor, Washington DC Chicago, Illinois (DuPage and Cook Counties only) New Orleans, Louisiana

*All lodging and meeting space must be in same hotel.

*Accommodations must have full service restaurant on site.

The U.S. Census Bureau reserves the right to conduct an on-site inspection of offered facilities. Lodging rate must be equal to or less than Government per diem rate. Interested sources must hold an active registration in the System for Award Management (SAM.gov), and provide full company name, DUNS# and Cage Code with proposal submission, and reference Solicitation #09-18-LRS-3 in email subject of all correspondence. All responsible sources from the five (5) cities solicited, that can meet the requirements and provide items as listed in the attachment may respond to this solicitation by submitting cost and technical proposal, hotel contract, and descriptive literature via email to lisa.r.safewright@census.gov, by 11:59 pm, Eastern Time on Wednesday, June 6, 2018. Please email all questions to the email address above. No phone calls will be honored. Award will be made on a best value basis, by Government Purchase Order, according to Simplified Acquisition Procedures. *Note, Weather/Travel/Airfare pricing/proximity from Regional Offices, will be factors in making the hotel and city selection.
