About this file

The document is a California Department of General Services Civil Rights Laws Attachment (form DGS OLS 04), specifically associated with Agreement Number 25WS0024, requiring certification of compliance with California civil rights laws for state contracts valued at $100,000 or above. The form mandates that contractors certify compliance with the Unruh Civil Rights Act and the Fair Employment and Housing Act for contracts executed or renewed after January 1, 2017.

The certification requires contractors to affirm that they do not have internal policies discriminating against sovereign nations or peoples recognized by the United States government, and that any such policies would not violate civil rights statutes. The document provides a standardized certification section for contractors to complete, including spaces for the proposer/bidder firm name, federal ID number, authorized signature, printed name and title of the signing person, county and state of execution, and date of execution.
