About this file

This is a Schedule B Pricing form for Request for Proposal No. 071I issued by the State of Michigan Department of Transportation (MDOT) for concrete patch supplies along I-94 eastbound between mile markers 23 and 27 in Sawyer. The proposal solicits pricing for four primary supply items: curing compound (20 five-gallon pails), epoxy coated smooth dowels (600 units at 1¼" x 18"), dowel caps of metal crimp type (200 units at 1"), and two-part epoxy with nozzles (25 53-ounce tubes). The first three items are not brand-specific and must comply with MDOT 2012 Standard Specifications for Construction, with references provided to specific sections covering curing compound (Section 903.06), epoxy coated dowels (Section 914.07, part A), and dowel caps (Section 914.07, part C). The two-part epoxy is brand-specific, requiring Adhesive Technologies Ultrabond product.

Contractors must submit pricing schedules in modifiable format (Microsoft Word or Excel) with all-inclusive cost pricing covering any one-time charges, setup fees, shipping, handling, and per-piece pricing. The proposal encourages submission of quick payment discount terms expressed as a percentage discount off invoice if paid within a specified number of days after receipt of invoice. Contractors must certify that prices were developed independently without consultation or agreement with competing contractors. Failure to complete the pricing schedule as requested may result in proposal disqualification.
