About this file

This is a Worker Protection and Investment Certification Form required by the Commonwealth of Pennsylvania for contractors and grantees working with the Department of Conservation and Natural Resources (DCNR), Bureau of Forestry. The form establishes mandatory compliance requirements with Pennsylvania's labor and workforce safety laws as mandated by Executive Order 2021-06. Contractors must certify compliance with thirteen specific Pennsylvania statutes and regulations, including the Construction Workplace Misclassification Act, Employment of Minors Child Labor Act, Minimum Wage Act, Prevailing Wage Act, Equal Pay Law, Wage Payment and Collection Law, and the Construction Industry Employee Verification Act, among others. The certification must be executed by an authorized official of the contracting entity and submitted to the Commonwealth's Purchasing Agency.

The form requires certifying officials to confirm their authority to execute the certification on behalf of their organization and to attest that the contractor or grantee maintains compliance with all applicable Pennsylvania state labor and workplace safety laws. Contractors must also report any changes in compliance status to the Purchasing Agency immediately upon occurrence. False certifications carry significant penalties, including contract termination and three-year ineligibility to bid on Commonwealth contracts under 62 Pa. C.S. 531, as well as prosecution under 18 Pa. C.S. 4904 for unsworn falsification to authorities. This certification is a prerequisite requirement for the DCNR Forestry Operations and Recreation dump truck rental opportunity.
