About this file

The document is an Affirmation and Disclosure Form, revised in March 2022, designed for state contract compliance. The form requires contractors to disclose the locations where services will be performed and where state data will be stored, with specific emphasis on prohibiting offshore services, locating state data offshore, or purchasing from Russian institutions or companies. Contractors must provide detailed information about their principal business location, service performance locations, and data storage locations, including those of any potential subcontractors.

The form mandates that contractors affirm their understanding of Executive Orders related to service locations and agree to immediately notify the state of any changes in service or data storage locations. Failure to provide complete location information may result in contract non-award. The state retains the right to immediately terminate the contract if services are moved outside the United States without a signed waiver. The form requires an authorized contractor representative to sign, acknowledging that the disclosure is part of the potential contract and that they understand the location and data storage restrictions.
