Attachment A: Design-Build Entity (Team Structure and Participating Company Profiles) [Replace these instructions with additional narrative details regarding the Design-Builder entity, including specific roles within its team structure (e.g., split of Project responsibilities and past working relationships among members of the design-build entity). For JVs or partnerships, provide a description of the agreement among the parties that clarifies intended risk sharing and intentions for joint and several liability. Include a team structure (companies) organization chart clearly showing Design-Build entity member(s) and their relationships to additional participating firms Provide detailed company descriptions for members of the Design-Builder entity (limited to two pages per each company). Do not include descriptions for additional participating firms (e.g., subconsultants/subcontractors) except for the Design-Builder’s lead design entity (if not part of the Design-Build entity itself).]
Attachment B: Design-Builder’s Delivery Team (Key Personnel and References) [Replace these instructions with resumes (up to two pages each) for each designated Key Position as required in Part 3 of the required RFQ response narrative. Each resume should include, at a minimum:
A summary of qualifications, including professional designations and registrations;
For experience shown on the resumes, describe the relevance for the responsibilities on this project;
Provide two project-related references for each Key Person (references should include the project name; contact person and her role for the relevant time of the reference; and a current working email address and phone number). All references may be contacted. Unresponsive references may negatively affect evaluations; and Cross-reference each Key Personnel resume to the projects listed in Appendix D.
Resumes should be included for all required Key Positions, plus any optionally designated Key Positions as per the RFQ instructions. Do not include resumes for Other Named Personnel or any other positions shown in response to Part 3 of the RFQ response.]
Attachment C: Design-Builder Relevant Project Experience Summary (People and Project Experience Table) Complete the table included on the following page as per the instructions herein.
The Project Relevance Criteria for Columns A through F on the following form are as follows:
Project Criterion A: Treatment design and construction projects (water treatment preferred, wastewater treatment acceptable) of similar size using membrane technology, preferably delivered using Design-Build or other Collaborative Delivery Methods.
Project Criterion B: Treatment of similar quality raw water influent for drinking water purposes, encompassing vendor selection and application of membrane technologies.
Project Criterion C: Supporting a public owner in identifying sources, supporting applications for, and obtaining funding to support the completion of an infrastructure project, preferably grants or low-interest loans from public sources.
Project Criterion D: Using a design-to-budget process to meet an objective for early cost certainty while also agreeing to performance requirements and acceptance criteria.
Project Criterion E: Working under an open book estimating process with frequent cost updates, followed by successful agreement on a price and contingency.
Project Criterion F: Transitioning an existing water system source (e.g., groundwater) to a new source (e.g., surface water) while evaluating and minimizing impacts to the distribution system pipelines.
Project Criterion G: Construction of complex water/wastewater facilities outside of urban areas and under extreme weather conditions, especially cold temperatures.
Project Experience Matrix
· Featured Projects listed in this Experience Matrix will be evaluated based on demonstrated relevant experience for the required Criterion A through G.
· Provide a minimum of three (3) and up to seven (7) reference projects for this matrix.
· Individual projects with alignment to the greatest number of relevant experience categories, supported by corresponding details in the project descriptions, will receive the highest scores.
· Only include information for subconsultants/ subcontractors that are members of the Design-Build entity for this project.
· Checking all the required experience categories with fewer projects is generally preferred.
· Caution: do not indicate relevance unless significant scope and experience is described in the accompanying project description.

Attachment D: Design-Builder Relevant Project Experience Detail (Relevant Project Descriptions and References)
[Note to Respondents: Replace these instructions with project descriptions for the projects listed in Attachment C, including at least the following information, limited to a maximum length of four pages each. The content of the resumes should be focused on the key relevance criteria shown in Attachment C.] Project Name and Brief Description:
Provide the project name and its location or address, the name of the owner, a one-paragraph description of the project scope, the total installed cost for the project, and your firm’s primary office location for this project, if different.
Delivery Method and Respondent’s Role:
Describe the primary project delivery method for the project [e.g., design-bid-build, CMAR or CM/GC, progressive design-build, fixed-price design-build, etc.] and your firm’s specific role on the project, including the total value of your contract/scope [e.g., as Design-Builder, General Contractor, Subcontractor, Design-Builder, or design/professional services].
Dates of Performance of the Work:
Provide a list of relevant milestone dates for the project [e.g., design-build notice to proceed, CMAR or progressive design-build phase 1 NTP, Phase 2 NTP, Substantial Completion, Final Completion]. If the project is not substantially complete, provide anticipated completion date and justification for using an incomplete project for this submittal.
Key Personnel:
Identify any proposed Key Personnel that were meaningfully involved with the work in the project description on that are proposed on project, as listed in Attachment C. Indicate (1) their position(s) on Project description’s team, (2) their primary physical location during the delivery of this Project description, (3) their expertise and experience, and the relevance to their proposed role(s) on this project.
Project Details:
Provide any additional project details, including photos, that relate to the project evaluation criteria similar characteristics.
Reference:
Provide a reference contact for each Project Description, including (at a minimum): name and role on the Project description (e.g., client, project manager, design manager, etc.), a current working phone number, and email address. All references may be contacted.]
Attachment E: Essential Requirements for Qualification and Certification (Pass/Fail Checklist and Required Documentation) All questions in this section are Pass/Fail:
The Design-Builder will be deemed unqualified if the answer to any of questions in E.1 through E.2, and E.4, including any subparts (i.e., E.1.a, E.1.b, etc.) is "No."
The Design-Builder will be deemed unqualified if the answer to any of questions in E.3 is “Yes.”
Where attachments are required to support the responses to the questions in Attachment E, include the required documentation at the end of Attachment E, in the order requested (except for financial statements, which should be submitted separately via BidNet). The City reserves the right to ask for any additional information based on the responses provided, and will use this information, along with any other relevant information, to evaluate whether or not the Design-Builder is qualified to perform the proposed work.
E.1 Insurance. Design-Builder has submitted a statement from an insurance company confirming that the Design-Builder or one of its team members has the capacity to provide the required insurance (as listed in the RFQ Appendix A) (“No” Answers will be disqualified):
