About this file

This is an amendment to the scope of work issued by the Ohio Department of Natural Resources Division of Oil & Gas Resources Management for the Carroll #8 Project at the Atwood Beach #1 Orphan Well Site in Carroll County, Monroe Township. The amendment increases the steel cable size specifications from the original requirement to 1/2" diameter for road mats, steel cable, and steel cable slings. The project involves installation of composite road matting (10,374 sq ft), steel cable (850 linear ft), and steel cable slings (55 each) to access and plug an orphan well. Critical materials include non-permeable composite mats at least 4 inches thick and 7'x13' in size, galvanized steel wire rope with 11.9-ton breaking strength, and 3-foot single leg cable slings. Offers must be submitted through OhioBuys by December 19, 2024 at 12:00 PM.

The project includes detailed specifications for site preparation, material installation, and safety requirements including anchoring systems within Atwood Lake's flowage easement. Additional contingency items are included for extended project durations or unforeseen conditions, with road mat costs to be provided per mat per day. The site plans show specific layouts for matting installation, cable anchoring points, and erosion controls. The work must be coordinated with lake water levels and includes provisions for proper demobilization if water levels rise. Site restoration requirements include specific quantities of seed (25 lbs), fertilizer (50 lbs), and mulch (6 bales) for disturbed areas.

SCOPE OF WORK

Atwood Beach #1 Orphan Well Site Carroll County, Monroe Township

DETAILED SPECIFICATIONS

The Contractor is reminded to review the Scope of Work documents carefully. Coordination, permission, or direction of the Division may be required for use of individual Detailed Specification line items. The Division shall only pay for quantities of items that are correctly installed and completed in accordance with the Detailed Specifications and Drawing Plan Set. The Division shall not guarantee payment of any work completed without or prior to following the conditions described herein of each line item.

ROAD MATS

A. Description: This item shall consist of the transportation, delivery, installation, and removal of road mats as described. The placement of road mats within the limits of construction shall be as shown on the Drawing Plan Set. This item shall be utilized to traverse the existing beach to the well site.

B. Material: Road matting shall be non-permeable, composite mats. Non-permeable, composite mats shall be a minimum of four (4) inches thick with a minimum surface dimension of seven (7) feet wide and thirteen (13) feet long. Non-permeable, composite mats and associated components (i.e.

ramps, berms, and fittings) shall be installed per the manufacturer’s recommendations with the exception that all mats shall be anchored using a combination of minimum 1/2" diameter galvanized steel wire rope placed through slings attached to all the mats along the outer perimeter (minimum of one (1) opening per mat) and tied back to on-shore anchor points. In addition, each interlocked mat shall be fully pinned.

All materials delivered to the site must be in a shape to be able to cover the area properly and still have the strength and integrity to complete the required work. The Division may reject any mats determined to be damaged beyond useful life or remove square footage as measured from each individual mat.

Requirements for the anchoring are specified under the “1/2" Steel Cable & 1/2" Steel Cable

Slings” line item.

C. Execution: Mats shall be kept clean throughout the project. If it is determined by the Division, the mats do not meet this requirement the Contractor shall have any sediment or mud removed immediately.

No mats shall be installed within the flowage easement without written approval from the

Division. All mats installed in this easement shall be anchored to shore as shown the Drawing Plan Set and as described in the Detailed Specification.

Mats shall be installed in a continuous fashion until all mats have been successfully placed. It is the

Contractors properly prepare the beach subgrade for the placement of the mats. This shall include but not limited to removal of stumps, trees, branches, trash etc. Preparation of the beach subgrade shall be considered incidental to this line item.

D. Measurement: Measurement for payment for the road mats shall be made by actual field measurements of quantities satisfactorily installed at the site. Each road mat shall be measured for a square foot installed.

For circumstances in which the Division extends the projected completion date (i.e. well obstructions, required milling operations, etc.), the Contractor shall also provide a cost for "Road Mats" as a dollar amount per mat per day under “Contingency Specifications” within the original Offer. Additional payment will be evaluated and determined by the Division.

In addition, a contingency line item for roads mats has been provided for circumstances where additional mats may be required due to unforeseen subgrade conditions. The Contractor shall be prepared to provide additional cabling and slings as required based on this quantity.

E. Payment: The cost of this work shall be included in the unit price per square foot for "Road Mats

(Composite)."

1/2" Steel Cable & 1/2" Steel Cable Slings

A. Description: This work shall include all labor, materials, and equipment necessary to deliver and install steel cable and steel cable slings for the purposes of providing anchor points on the composite matting within the flowage easement of Atwood Lake.

B. Materials:

a. Steel Wire Cable: Cable shall be at minimum 1/2" diameter, 6x19 galvanized wire rope constructed of 6 strands and 19 wires per strand with a minimum breaking strength of 11.9 tons and working load limit of 2.38 tons. It is the Contractor’s responsibility to provide the appropriate quantity and fittings required as described in this specification as well as shown on the Drawing Plan Set. A potential supplier is Weisner Inc., 1 (800) 535-0347.

Cabling directly in contact with the anchoring points shall be vinyl/rubber coated so as to minimize wear. The Contractor shall be responsible for determining the product and working with the Division for approval.

b. Cable Slings: The cable slings shall meet the minimum material specifications as required for the steel wire cable. Slings shall be a single leg style at a minimum of three (3) foot in length. A potential supplier is Alp Industries, www.alpindustries.com.

c. Fittings: 1/2" galvanized drop forged wire rope clips shall be used when connecting cable end to end and or when looping cable. In addition, 1/2" metal stops/ferrules shall be used at all termination ends on cabling. The number of clips and stops/ferrules shall be the responsibility of the Contractor. All fitting shall be meet the minimum performance requirements of the 1/2” cabling.

Note: Due to the potential for variation in cable stretch, slack and or the need for additional matting an excess quantity of approximately 5% for cabling and slings has been accounted for on the quantity sheet.

C. Installation: A surveyor from the Division shall stake the location for each anchor point. At no point shall matting be left unanchored within the flowage easement.

http://www.alpindustries.com/

1. General: All materials shall be handled, stored, and installed in accordance with the manufacturer’s recommendations.

2. Steel Wire Cable and Slings: Each anchor point shall have an associated cable spool. Cable shall be run simultaneously on each side of the mats as they are installed. Cable shall be placed through slings attached to all the mats along the full outer perimeter (minimum of one (1) opening per mat) and tied back to anchor points as well as anchored to the well casing as shown on the Drawing Plan Set.

At the end of each workday, cable ends shall be connected so that the mats are not left unanchored. When connecting cable, an 18” minimum overlap is required and shall be bound using wire rope clips and terminated ends shall be bound using stops/ferrules as shown on the Drawing Plan Set.

Once the project is complete, the Contractor shall remove the steel cable & steel cable slings.

All cabling and slings shall become property of the Contractor upon completion. No additional payment will be made for materials not utilized.

D. Measurement: Measurement shall be based on the actual linear feet of steel cable and each cable slings installed and accepted.

E. Payment: Payment for this work shall be made at the unit price per linear foot of "1/2” Steel Wire Cable" and unit price per each of “1/2" Steel Cable Slings.”
