1. Introduction: This Quality Assurance Surveillance Plan (QASP) has been developed to ensure the performance of critical contract elements. The QASP serves as the plan for surveillance of performance and identifies the performance indicators, standards, inspection methods and procedures to be used in monitoring performance. Additionally, the QASP shall include specified procedures for collecting service delivery data, methods of surveillance, thresholds for acceptable and unacceptable performance and sampling guides.

Overview. This Quality Assurance Surveillance Plan outlines the procedure for surveillance of the contract. Included are procedures for evaluating the contractor’s performance and how key services will be inspected.

Purpose. The purpose of the QASP is to show in detail the government’s plan for monitoring the performance of the contractor and to provide for quality assurance. The QASP has been developed to evaluate contractor actions while accomplishing the Performance Work Schedule (PWS).

2. Responsibilities: The contractor is responsible for management and quality control actions to meet the terms of the contract. The role of the government is quality assurance to ensure contract standards are achieved.

2.1. The government desires to maintain a quality standard that meet the requirement of the contract. The government is responsible for Quality Assurance and not Quality Control.

Surveillance: The government POC will evaluate the services required to ensure compliance of services provided.

Standard: 100% of the work performed will be satisfactorily completed without customer complaint and/or rework.

Procedures: The government POC will inspect work required in order to ensure contractor’s compliance with the appropriate paragraphs of the PWS. The POC will record results of inspection, noting the date and time of inspection. If the inspection indicates unacceptable performance, the POC will inform the Contracting Officer (KO) of the deficiencies for correction. The contractor shall be given a reasonable amount of time after notification to correct the deficiencies.

Primary Method of Surveillance: The primary method of surveillance describes which surveillance method will be used to monitor the contract requirement. This element also indicates whether the surveillance method will be supported by validated customer complaints, incidental inspections or alternate methods. There is one method used to evaluate the contractor’s work: 100% inspection of the 4-hour training session.

Evaluation Procedure

1. Primary Function: The POC has a primary function to evaluate the performance of the contractor. In order for evaluations to be conducted in an efficient and accurate manner, and minimize the opportunity for a dispute, it is very important the POC follow certain procedures when scheduling, conducting and documenting inspections.

2. Evaluation Worksheets: As a minimum, the Evaluation Worksheet should contain the PWS reference numbers, location, and date of the inspection. Regardless of how the evaluation is conducted, the Evaluation Worksheet should be completed during or immediately after the inspection. A clearly written record of the findings is the Government’s primary evidence in the event of a dispute.

3. Evaluation of Performance: At the end of the evaluation period, the COR should summarize all the information on the Evaluation Worksheets and determine the total number of deficiencies for each required service. Any performance not in conformance with standards will be classified as a defect.

4. If the contractor performance becomes “other than satisfactory” the COR will issue a Contract Deficiency Report (CDR) on DA Form 5479R to the KO for action.
