Proposers must also complete conflict of interest disclosures as required by ODOT's COI Guidelines and either certify full compliance with no conflicts or provide detailed disclosures on separate TGM COI Disclosure forms. The authorized representative of the proposing firm must sign and date the cover sheet to certify they are duly authorized to bind the proposer and make all representations contained in the proposal.
