About this file
This document is an OSHA Certification Requirement form for public building and public works projects in the City of New Bedford, Massachusetts, governed by Massachusetts General Laws Chapter 30 §39M and Chapter 149. The certification mandates that all contractor employees working on public works sites must have successfully completed a minimum 10-hour OSHA-approved construction safety and health course, effective July 1, 2006.
The form requires contractors to certify their employees' safety training by submitting OSHA completion cards for each worker to the City of New Bedford before project commencement. Contractors must provide these documentation cards initially and continue to supplement them as new employees are hired or contracted to work on the project. The document is a standard compliance certification that must be signed by an authorized representative of the bidding entity, including their name, position, company, and the date of certification.
