THE MT HCCN FILLS A CRITICAL GAP IN THE HEALTH INFORMATION TECHNOLOGY RESOURCES OF ITS NETWORK MEMBERS WHILE SUPPORTING THEM THROUGH INTENSIVE COACHING AND TECHNICAL ASSISTANCE TO SUCCESSFULLY AND SECURELY DEVELOP AND DEPLOY DIGITAL CARE STRATEGIES TO IMPROVE ACCESS, IMPROVE PATIENT OUTCOMES, MAINTAIN PATIENT CENTERED MEDICAL HOME RECOGNITION, CONTINUE INTEGRATION OF BEHAVIORAL HEALTH (MENTAL HEALTH AND SUBSTANCE USE) SERVICES AND SUCCESSFULLY EXPAND THE NETWORK'S PARTICIPATION IN VALUE-BASED CARE.
