THE AHCCN WILL COLLABORATE WITH FEDERAL, STATE, AND LOCAL PARTNERS INCLUDING HRSA/HCCN, NATIONAL COOPERATIVE AGREEMENTS (HITEQ, NACHC, AND OTHER NTTAPS), ARKANSAS OFFICE OF PRIMARY CARE, STATE HIE, ALEDADE (ACO), AND OTHERS. THE T/TA ACTIVITIES PLANNED INCLUDE: 1) ADVANCING PATIENT ENGAGEMENT THROUGH IMPLEMENTATION OF VARIOUS INTEGRATED DIGITAL HEALTH TOOLS INCLUDING PATIENT PORTALS, BIDIRECTIONAL DIGITAL MESSAGING, REMOTE PATIENT MONITORING, AND VIRTUAL CARE/TELEHEALTH THROUGH ASSESSMENT, TRAINING, IMPLEMENTATION, AND OPTIMIZATION OF TOOLS; 2) ENSURING PATIENT PRIVACY AND CYBERSECURITY THROUGH ASSESSMENT AND EVALUATION OF SYSTEMS, PROVIDING TOOLS AND RESOURCES TO ESTABLISH COMPREHENSIVE RISK MITIGATION PROCESSES, AND ASSISTING WITH ADOPTION OF INDUSTRY BEST PRACTICES; 3) SOCIAL DETERMINANTS OF HEALTH RISK FACTOR INTERVENTION THROUGH ENHANCING IT CAPTURE OF DATA, REPORTING, AND ANALYSIS; 4) DISAGGREGATED PATIENT LEVEL DATA REPORTING THROUGH READINESS OF UPLOAD UDS+ FHIR BASED APIS; 5) INTEROPERABILITY AND DATA INTEGRATION THROUGH ASSESSMENT AND IMPLEMENTATION OF HIE CONNECTIVITY PROJECTS; 6) DATA UTILIZATION THROUGH VISUALIZATION, ANALYSIS AND IMPLEMENTATION PROJECTS; 7) LEVERAGING DIGITAL HEALTH TOOLS BY ASSESSMENT AND IMPLEMENTATION OF TRAINING PROGRAMS TO INCREASE UTILIZATION BY PROVIDERS AND STAFF; 8) HEALTH IT ADOPTION THROUGH FURTHER TRAINING ON USE OF EHRS BY VENDORS AND TRAINING TO PROVIDERS AND STAFF AND 9/10.)
