Description
State maternal health innovation program - this project responds to funding opportunity number: hrsa-24-047 with a purpose to reduce maternal mortality and severe maternal morbidity (SMM) through improved access to care, enhanced data surveillance and implementation of innovative interventions to improve outcomes for populations disproportionately impacted by maternal mortality and SMM. To accomplish this, we will create the pennsylvania public-private perinatal action collaborative (PAC) which will function as a maternal health task force. The PAC will cultivate cross sector partnerships to improve maternal health outcomes through coordination of state maternal health programs, enhanced data surveillance systems, guiding implementation of innovative care models in highest risk counties and utilizing continuous quality improvement (CQI) processes to build more effective and sustainable infrastructure. The PAC will partner with the jewish healthcare foundation for organizational support to establish the primary PAC committee as well as a committee of community members with lived experience who will be represented in all aspects of the project. The PAC will identify needs areas, develop a maternal health strategic plan and identify a select number of counties with highest rates of maternal mortality. The PAC and PAC intervention teams will coordinate a multidisciplinary intervention with each county and community-based organizations in those regions to implement an innovative care model that addresses care gaps identified. The PAC will partner with the implicit network which has created, implemented and scaled care models which have the potential to improve maternal health through screening and service integration. Health systems and counties who receive these interventions will become incorporated into a community of practice to receive ongoing training and support to sustain care models over the long term. Using a newly created maternal health dashboard, the PAC will track performance O N key maternal health indicators including postpartum care engagement. Through partnerships with the mixed methods research lab and amethist at the university of pennsylvania, qualitative and implementation science evaluation of current and future processes will occur to help inform and improve performance in these areas. The PAC will work to achieve 4 main objectives including improved coordination of maternal health programs, reduction in racial disparities in maternal mortality and SMM, improved quality of care through innovation, and enhanced data capacity.
