Description
Primary care training and enhancement - residency training in mental and behavioral health - recognizing that residency is the critical learning period as normative practices of care are set, waco family medicine's primary care training and enhancement program will cultivate and expand behavioral health competencies during residency, increasing the quality of these services provided by family medicine physicians. Our residency training occurs within a primary care behavioral health innovation hub that produces and maintains nationally recognized decision support tools; offers apprenticeship-style training for behavioral health in a dedicated rotation with a clinical psychologist in CO-visits along with real-time supervision from family medicine faculty; features primary care addiction medicine experience; and provides continuity training with integrated behavioral health providers. This program includes maintaining and augmenting human behavioral and mental health and outpatient pediatric experiences through scheduled clinical rotations at waco family medicine (WFM) clinics predominantly serving under-served children and adolescents. In addition to these experiences, WFM will add new external clinic sites through expanded partnerships with community behavioral health/primary care organizations. Residents will interface with specialty behavioral health care through clinical training with a child and adolescent psychiatry as well as a crisis intervention. Building on our current family medicine residency foundation, this project will expand the current medication for opioid use disorder (MOUD) to include treatment for other substance use disorders (stimulant, tobacco, alcohol use), interface with specialty behavioral health care – specifically child and adolescent psychiatry, and provide crisis intervention clinical experience with our local mental health authority. In addition to the time-bound clinical programs, there will be formal training in team-based care in residency continuity clinics to grow didactic and team-based instru ction in high-functioning aspects of teams for residents and staff. We also intend to expand didactic and clinical instruction in measurement-based care. Deliverables include the following: - behavioral health and pediatric outpatient rotations, child and adolescent psychiatry and primary care addiction medicine clinical experiences - provide trauma-informed care and gun violence prevention/response training experiences - team-/measurement-based care workshop sessions (using purpose built curricula) - decision support tools specifically addressing adolescent patients measurables/data tracked will include: 1) resident participation throughout their training period data (start date, completion of program components, graduation date, duty hours reporting for various clinical activities related to the project) 2) workshop information (titles, presenters, dates, and attendees) 3) resident in training exam scores for behavioral health domains 4) developing and tracking enduring educational content 5) graduate employment data (location, practice type, MUC, and hpsa statuses). The program and its data tracking will be led by zach sartor, MD, with behavioral health and clinical support from additional key/senior faculty, staff, and experienced, recognized consultants. Most proposed program alterations will occur internally within the waco family medicine system. All the training experiences proposed will focus on the prevention, identification, diagnosis, treatment, and referral of services as necessary for behavioral health disorders throughout the lifespan - including children, adolescents, and young adults. The learning process will primarily be experiential, with intentional didactic instruction from within our organization and in partnership with others to achieve the training necessary to provide prevention, identification, diagnosis, and treatment of behavioral health conditions. WFM requests funding priority under qualification 1: high MUC placement rate .
