The Cahaba Medical Care Foundation has been awarded a $1,000,000 Project Grant under the Affordable Care Act (ACA) Grants for New and Expanded Services Under the Health Center Program (CFDA 93.527) from the Health Resources and Services Administration (HRSA). This funding will support Cahaba Medical Care's initiative to enhance and expand services aimed at facilitating the transition of justice-involved individuals reentering the community (JI-R) in Bibb County, Alabama.
The project will implement a comprehensive care transition program to improve access to community-based, high-quality primary care services within 90 days of release from incarceration. Key activities include conducting comprehensive health assessments, developing personalized care plans, offering behavioral health support, and assisting with access to essential social services. By addressing both healthcare and social determinants of health, the program aims to reduce recidivism and promote long-term health and stability for the JI-R population. The project will run from December 1, 2024 to November 30, 2026.
