About this program
The Department of Health and Human Services Health Resources and Services Administration (HRSA) funds transitions-in-care programming for justice-involved individuals reentering the community from incarceration, with the most recent grant awards disbursed December 1, 2024, under Assistance Listing 93.527.
The program funds pre- and post-release health care coordination and primary care services for individuals within 90 days of release from carceral facilities. Funded activities include chronic disease management, screening and treatment for infectious diseases (hepatitis C, HIV, sexually transmitted infections), substance use disorder treatment and medication-assisted treatment, mental health services, overdose prevention, care coordination through case management, and wraparound support addressing health-related social needs such as housing, food insecurity, transportation, and financial strain. Award recipients establish partnerships with jails and prisons to provide in-reach services during the final months of incarceration and facilitate seamless transitions to community-based primary care upon release.
The portfolio comprises 5,049 awards since 1983, predominantly Project Grants (4,977 awards, 99 percent) with a small share of Cooperative Agreements (72 awards, 1 percent). Recent funding activity has been robust: 1,501 awards in 2023 and 1,498 in 2022, though awards declined to 54 in 2024. Combined federal dollars obligated total $37.39 billion. HRSA funds the program through two operational channels within the same agency, accounting for the split in top-funder reporting: one channel awarded 1,681 grants totaling $35.1 billion in federal dollars, the other 3,368 grants totaling $2.29 billion. Recent award magnitudes range from $1 million to $6 million, with typical awards in the $2 million to $4 million range over two-year project periods.
Representative recent awards include Caring Health Center in Massachusetts ($2 million) to provide expedited health care access and integrated medical, behavioral, and social care for post-release justice-involved individuals; Sterling Health Solutions in Kentucky ($3 million) to expand transitional health care services for incarcerated individuals within 90 days of release; and Erie County Health Department in Ohio ($4 million) to establish coordinated care and wraparound support for justice-involved adults transitioning back into the community. The majority of funded health centers are federally qualified health centers (FQHCs) serving rural and underserved urban populations.
HRSA posted a competitive opportunity (HRSA-25-005, Quality Improvement Fund - Transitions in Care for Justice-Involved Populations) with a due date of June 10, 2024. The solicitation restricted funding to existing Health Center Program award recipients and required that activities focus solely on engaging justice-involved individuals within 90 days of release, with funds not to be used to replace carceral authorities' obligations to provide medical care or to serve individuals more than 90 days from scheduled release.
