RESIDENTS OF THE SERVICE AREA HAVE A LOWER MEDIAN INCOME ($33,747 VS. $55,454 STATEWIDE), LOWER LEVELS OF EDUCATION (JUST 74.4% HAVE A HIGH SCHOOL DIPLOMA OR HIGHER COMP ARED TO 87,7% STATEWIDE). THERE IS A LOWER LEVEL OF IN-HOME INTERNET CONNECTION WHICH PREVENTS ACCESS TO INFORMATION ACROSS THE SERVICE AREA. RESIDENTS OF THE SERVICE AREA FACE POORER HEALTH OUTCOMES THAN PEOPLE ACROSS THE STATE, HIGHER RATES OF PREMATURE DEATH, AND A HIGHER NUMBER OF POOR MENTAL HEALTH AND POOR PHYSICAL HEALTH DAYS EACH MONTH. VACCINATION ACCEPTANCE IS LOWER IN THE SERVICE ARE WITH THE RATE OF VACCINATION FOR COVID-19 OVER 10% LOWER THAN THE REST OF THE STATE. OVERALL, RESIDENTS OF THE FOUR-COUNTY SERVICE AREA ARE FACING INCREASE HEALTH DISPARITY AS COMPARED TO KENTUCKIANS IN GENERAL. ALL NETWORK PARTNERS HAVE SIGNED LETTERS OF COMMITMENT AND HAVE DEVOTED RESOURCES TO THE RECOVER KENTUCKY PROJECT ALL THREE NETWORK PARTNERS HAVE EXTENSIVE EXPERIENCE SERVICE THE TARGET POPULATION THROUGH THE DELIVERY OF PRIMARY CARE, SUBSTANCE USE SERVICES, HOSPITAL SERVICES, AND COMMUNITY HEALTH WORKER SERVICES. AS A RESULT OF THE PLANNING ACTIVITY, THE SERVICE AREA'S RURAL UNDERSERVED POPULATION WILL EXPERIENCE INCREASED ACCESS TO QUALITY SERVICES ACROSS THE CONTINUUM OF CARE.
