Description
Supporting health in all policies with social determinants of health accelerator plan - to close the GAP of disparities in health for chronic disease, the peoria, illinois leadership team will use a collaborative approach for supporting health in all policies with social determinants of health (SDOH) accelerator plans. The leadership team will leverage collective impact among government, service providers, multi-sector partners, and the community residents to position a health in all policies (HIAP) strategist to oversee and align community development plans with community health improvement plans. Adding a SDOH lens in all reviews of plans will help address the disparities related to healthcare and chronic disease by advancing a more equitable built environment and enhancing social connectedness in the community. The leadership team includes members of the health and human services subcommittee of a larger racial justice equity commission for peoria county. The subcommittee is composed of members from multi-sectors and the lived community. Additional leadership team members are from the lived community or representatives from organizations working in the targeted equity areas. The team will work with the strategist for these outcomes: 1. To increase multi-sector collaboration among healthcare, government, business, partner organizations, and community residents; 2. To develop a SDOH accelerator plan that identifies steps to assure policy development and coordination of cross-sector plans in the built environment and to assure a health lens and a SDOH/equity review during planning stages of these community plans; 3. To increase community social connectedness by increasing resident engagement to have a voice in the planning process and beyond; and 4. To increase sustainable strategies in the accelerator plan, so that the plan can continue to be used to address disparities related to SDOH and chronic disease, and also to address health equity in general. Addressing chronic disease through SDOH with more inclusive built environment policies for active multi-modal transportation, access to safe walkways to everyday destinations, and use of complete streets will assure communities better social connectedness to congregate, increase good mental health days, and increase the voice of advocacy by residents for inclusion policies in their own neighborhoods.
