Description

Eleven health care organizations came together to create the northland health alliance (NHA) with the idea to bring organizations and collectively address the health problems as a single unit. The NHA conducted a community health needs assessment in 2021 and is now tasked with the creation of the community health improvement plan (CHIP). The aim of the completed CHIP will be to accelerate the progress being made in combating the health inequities through a focused effort on the social determinants of health. Through facilitated meetings attended by representatives of each member organization, the NHA, along with other organizations with interest in the priority areas, will create a community health improvement plan (CHIP) to accelerate the improvement of the selected priority areas. The goal by the end of the project period is 1) have a completed plan to address a. The built environment, B. Social connectedness, and C. Food insecurity, and 2) increase collaboration among community partners in the northland who have a vested interest in improving the health of the community.
