The Continuum of Care Program provides funding for efforts by nonprofit providers, states, and local governments to quickly house homeless individuals and families while minimizing trauma and dislocation caused by homelessness, promote access to mainstream programs by homeless individuals and families, and optimize self-sufficiency among those experiencing homelessness. Eligible activities include permanent supportive housing for persons with disabilities, rapid rehousing, transitional housing, supportive services only, Homeless Management Information Systems (HMIS), and in some cases homelessness prevention. Funding may also support planning activities, United Funding Agency costs for fiscal control and accounting to subrecipients, acquisition and rehabilitation of real property for housing or supportive services, and new construction.
