About this award
The Department of Housing and Urban Development Assistant Secretary for Community Planning and Development awarded $541,880 to Alliance To End Homelessness on October 1, 2025, under the Continuum of Care Program (CFDA 14.267) to operate the Entry Point Coordinated Entry/Domestic Violence system serving individuals and families who are victims of domestic violence, dating violence, sexual assault, or stalking and who are at risk of or experiencing homelessness in Cook County, Illinois.
The system deploys cross-trained coordinated entry navigators co-located at victim service agencies, in the field, at hospitals, courthouses, community locations, evening emergency shelters, and daytime support centers. Alliance implements a county-wide coordinated intake, assessment, safety planning, and housing placement process utilizing a universal intake form, standardized prioritization, and a single by-name list matching client needs with appropriate housing resources. Street outreach programs prioritize the most vulnerable unsheltered homeless individuals and subpopulations including chronic, veterans, and unaccompanied youth. The award funds coordination between the Continuum of Care and victim services systems across nonprofit homeless providers, victim service providers, faith-based organizations, governments, businesses, public housing agencies, school districts, mental health agencies, hospitals, law enforcement, and organizations serving veterans.
Performance period runs through September 30, 2026, with place of performance in Hillside, Illinois. Sub-recipients participating in Entry Point system coordination include Pillars Community Health, Wings Program Inc., YWCA Evanston/North Shore, Sarah's Inn, Anew Building Beyond Violence And Abuse, and Crisis Center For South Suburbia, each deploying navigators, coordinated intake processes, and safety planning functions across regional service sites.
