The U.S. Department of Housing and Urban Development (HUD) awarded a $3,621,787 Continuum of Care (CoC) Program grant (CFDA 14.267) to the Christian Community Health Center (CCHC), a minority-owned non-profit organization in Chicago, Illinois. The grant aims to promote community-wide efforts to end homelessness by providing funding for projects that quickly house homeless individuals and families while minimizing trauma and promoting self-sufficiency.
The CoC Program grant will allow CCHC to carry out a range of eligible activities, including permanent housing, transitional housing, supportive services, homeless management information systems, and potentially homelessness prevention. As a sub-grantee, CCHC has previously collaborated with the City of Chicago and the Chicago Department of Public Health on initiatives focused on HIV/AIDS housing needs and enhancing public health agencies' capacity to respond to infectious diseases. This three-year award, effective from March 2025, will enable CCHC to continue its work in addressing homelessness and providing comprehensive healthcare and social services to underserved populations in the Chicago area.
