Description
Ontrack rogue valley?s residential treatment home program for pregnant & postpartum women (PPW) and their minor children - project: ontrack rogue valley's residential treatment home program for pregnant & postpartum women (PPW) & their children. Summary: home is a 24-bed SUD residential treatment facility for moms with their children ages 0-5 designed to keep families united. Home provides comprehensive SUD treatment, recovery support services, healthcare, mental health services, & harm reduction interventions for PPW across a continuum of specialty SUD residential care, based on comprehensive, individualized screenings & assessments that inform treatment planning & service delivery in a continuous care model using evidence-based (EB) practices. Prior to intake & after discharge, PPW have access to outpatient care & services that address social determinants of health. Using a holistic approach, home will provide supplemental services & interventions for minor children, partners & other extended adult family members (AFM) of the PPW & children, as requested by the PPW. Services & locations are all accessible for women with low incomes. Population served: home serves PPW from across oregon. On average, 75% are from jackson & josephine county, 87% are white, 7% hispanic/latino, 3% african american/black, 3% indigenous/native american. Nearly 100% speak english or are bilingual; less than 1% speak only spanish. The only SEX/gender criteria for admission is identifying as being a mother; ~5% identify as lgbtqia+. Ages ranges from 18-35; 98% are on medicaid; 25% are unhoused when entering treatment; 44% are pregnant; 88% reside at home program with their children. Strategies/interventions: home provides direct outreach to PPW with SUD, 24/7 staff, individualized multidisciplinary care teams, a certified child care center on site, & SUD treatment using EB practices & harm reduction services for PPW & their families. Under this project ontrack seeks to expand its partnership with OASIS-a health clinic serving parents with suds & their children-to enhance care coordination for PPW & their children. OASIS' medical team/OMT will provide primary medical, prenatal, postpartum & mat services (as needed) for PPW & pediatric care for PPW's children. Ontrack will hire a women's coordinator to increase care coordination, an access & utilization manager to ensure no services are denied due to lack of insurance, and a quality assurance medication manager to manage PPW's prescriptions. OASIS will hire an RN & peer support doula to support OMT & PPW, and OASIS' children's coordinator will provide developmentally appropriate child services & EB parenting training & interventions for PPW. To increase early engagement & reduce dropout early in the recovery process, ontrack & OASIS are partnering to add emergency housing for PPW on the home waitlist & child watch for outpatient treatment. Goals: 1) reduce substance use & the severity of addiction by providing low-barrier continuum of SUD treatment coordinated with recovery supports. 2) increase child well-being and reduce addiction-related out-of-home placements by providing age-appropriate services to the minor children and AFM, including primary and behavioral healthcare, parent training, and access to comprehensive social services. 3) improve outcomes related to social determinants of health (education, stable housing, employment, childcare) and support sustained recovery of PPW by providing recovery supports & resources for PPW. 4) increase access to harm reduction interventions for PPW including outreach, emergency housing, and FDA-approved overdose reversal medications. Outcomes: home will annually serve 30 PPW in residential treatment (to be included in evaluation); 45 minor children; 10 AFM; 30 additional PPW with outreach, outpatient treatment & harm reduction support. 575 total over 5 years.
