FRONTIER HEALTH-HEALTHY TRANSITIONS - THE GEOGRAPHIC CATCHMENT AREA WHERE HEALTHY TRANSITIONS SERVICES WILL BE DELIVERED IS FOUR COUNTIES (SULLIVAN, JOHNSON, HAWKINS, AND GREENE) IN THE NORTHEAST REGION OF TENNESSEE. THE POPULATION OF FOCUS IS YOUTH AND YOUNG ADULTS AGES 16-25 WITH SERIOUS MENTAL ILLNESS.
