Description

Two-generational mental health expansion - through our two-generational mental health expansion, mountain home will expand by 55% our needed mental health services to pregnant and parenting mothers, who are homeless or at risk of homelessness, and their children. Leveraging our proven wraparound model, we will deliver intensive outpatient clinical care to children and adults via our onsite, licensed mental health center, home visiting services and hipaa-compliant telehealth capabilities developed in 2020. Mountain home serves women (ages 14-29) and their children. More than 90% of the mothers in our care suffer from a severe mental illness (SMI). Self-medication and CO-occurring disorders (COD) are common; 65% of moms report personal histories of addiction. We priorities our children's mental health services for mountain home children struggling with severe emotional disturbance (SED) - often as a result of the trauma they have already experienced, such as homelessness, parental separation, and/or substance abuse or violence in the home. As part of this project, we will continue to offer therapy, support from a licensed addiction counselor, peer support, CBPRS support services, and case management for mothers. We will expand telehealth services, a service delivery model that has greatly benefited families during the pandemic. This project will also allow US to significantly grow our needed children's mental health services. Not only will mountain home more than double the number of kids served, we will add case management for children as part of our successful wraparound model of care. This game-changing grant allows US to create six new positions (that will bill medicaid for a portion of their hours) as well as expand the roles of current staff to ensure the success and sustainability of this significant program expansion. By october 2021, we will have hired the following new full-time positions: children's therapist, adult therapist, children's case manager, adult case manager, and community based psychiatric rehabilitation and support staff (CBPRS). By october 2022, we will hire our organization's second peer support specialist. With this expanded capacity, mountain home will serve 55% more individuals from 2020 levels - in year 1, serving 80 mothers and 25 children and, in year 2, serving 90 mothers and 35 children. Over the two-year period, we will serve 110 mothers and 40 children (UNDUPLICATED). This project will leverage population-appropriate evidence based models, such as motivational interviewing (MI), cognitive behavioral therapy (CBT), eye movement desensitization and reprocessing (EMDR), and infant-parent psychotherapy (IPP). Staff will be trained in samhsa's 6 principles of trauma-informed care and continue to integrate them into screening, assessment, diagnosis, and patient-centered treatment planning. To ensure sustainable services, our team will also be provided with ongoing self-care and mental health support. This thoughtful approach will ensure the project increases access to high-quality, effective care for vulnerable moms and kids from around our rural state during this critical period and well into the future.
