PROJECT WILL DIRECTLY EXPAND WCMH'S CAPACITY TO PARTNER WITH OUR LOCAL COMMUNITY-BASED ORGANIZATIONS FOR ADDRESSING A PATIENT'S SOCIAL DETERMINANTS OF HEALTH, INCREASE PATIENT SATISFACTION, AND EXPAND OUR HOSPITAL'S CAPACITY TO PROVIDE HIGH QUALITY PATIENT CARE.
