Description
Healthy homes initiative (HHI) in maricopa county project goals: the HHI's goals are to: 1. Eliminate health and safety hazards for medicare and medicaid recipients with chronic conditions whose housing and home hazards contribute to poor health outcomes; 2. Align work with current programs to eliminate duplication and to ensure whole house approach (WAP, liheap, low-income utility program); 3. Validate health care cost savings; 4. Disseminate proof-of-concept for replication across the U.S., and 5.identify potential sustainable funding mechanisms for non-energy health-related measures. Fslhi proposes to improve the health outcomes of a minimum of 100 households that are eligible and have requested WAP. Underserved households of medicaid-insured individuals with chronic conditions whose home environments exacerbate their medical conditions are the target population. The HHI will remediate health and safety hazards while leveraging committed funding to complete WAP in the homes. In addition to collaborations with existing WAP agencies, fslhi will partner with mercy care, an arizona medicaid insurer, and THREE3 INC., a nonprofit organization with expertise in evaluation and social science research, to analyze pre-/post intervention health and claims data and pre-/-post client survey responses to calculate the financial impact of the initiative as well projected quality of life outcomes. Fslhi and THREE3 will publish the pilot's outcomes and identify potential funding sources in order for WAP organizations and their partners to replicate this program throughout the country.
