THEY WILL PERFORM OUTREACH, ASSIST IN NAVIGATION OF SERVICES, AND PROVIDE COUNSELING AND COACHING TO UNDERSERVED POPULATIONS IN MONTROSE AND DELTA COUNTIES, CO; PINELLAS AND BREVARD COUNTIES, FL; COOK COUNTY, IL; JEFFERSON COUNTY, KY; HENNEPIN COUNTY, MN; GARDEN COUNTY, NE; SAN JUAN MUNICIPALITY, PR; AND DALLAS AND HARRIS COUNTIES, TX.
