COMMUNITY HEALTH CENTER, INC. (CHCI) PROPOSES TO HAVE 16 AMERICORPS MEMBERS WHO WILL CONDUCT OUTREACH, PATIENT ENGAGEMENT, COMMUNITY EDUCATION AND PREVENTION SERVICES IN HIGH-NEED COMMUNITIES ACROSS SIX STATES, INCLUDING CONNECTICUT, MASSACHUSETTS, NEW YORK, ARIZONA, CALIFORNIA, AND THE STATE OF HAWAII. AT THE END OF THE FIRST PROGRAM YEAR, THE AMERICORPS MEMBERS WILL BE RESPONSIBLE FOR SUPPORTING INCREASED HEALTH KNOWLEDGE AMONG INDIVIDUALS IN THE COMMUNITIES SERVED. IN ADDITION, THE AMERICORPS MEMBERS WILL LEVERAGE 28 VOLUNTEERS WHO WILL BE ENGAGED IN OUTREACH, PEER MENTORING AND YOUTH PREVENTION/DEVELOPMENT ACTIVITIES ALONGSIDE AMERICORPS MEMBERS.
