Description
The service area includes the counties of cass, hubbard, clearwater, and beltrami and includes the red lake, leech lake, and white earth reservations. SHBH's priority is promoting racial equity and removing barriers to access and opportunity for communities that have been historically underserved, marginalized, and adversely affected by inequality, specifically in our region those identifying as native american. Our region has a higher population rate of native americans with deep histories of historical trauma and lack of equity in resources and care, all of which has contributed to loss of cultural identity. With the overall goal of successful reentry, reduced recidivism, and strengthening communities. Deliverables will include recidivism rates that remain under 33% and increased collaboration with partners. Activities will include completion of LS-CMI on 75% of participants in the pre-release phase; cultural needs assessment on 90% of participants; and a comprehensive case management plan that aligns with needs identified through the LS-CMI and cultural assessment on 95% of participants; and participation in one culturally relevant activity by at least 50% of participants. Targeting a focus population of up to 200 individuals annually who recently have been released from incarceration and are returning to the north central region of minnesota. Sanford health behavioral health (SHBH), new beginnings program is connecting individuals with services and support they need for success. The new beginnings program seeks to assist participants in identifying their needs and connecting them with resources in order to start healing and rebuilding their lives. SHBH offers a range of mental health and substance use disorder (SUD) services available to those accessing the RE-entry program including rule 25 assessments, cultural assessments, needs assessments and intensive case management services. The new beginnings program also offers referrals and assistance with SUD treatment, peer recovery support services, mental health diagnostic assessments and counseling, cultural activities and healing practices, skills training, medical and/or dental care, transportation, and referral to housing programs. SHBH has recognized the need to increase our cultural competency in caring for american indian populations, and have improved our partnerships with tribal agencies. SHBH is requesting the priority consideration for care of native americans. Beginning in the summer of 2021, SHBH worked tirelessly to improve relationships with all local tribal entities to include red lake, leech lake and white earth.
