Description

Baltimore city: health equity diabetes - under component 2: use of multisectoral partner networks to scale and sustain the national DPP to reach priority populations, the baltimore city health department seeks $1,100,000 to reduce the risk of type 2 diabetes and address the disparity of severity and prevalence between black and white baltimore city residents by outreaching and enrolling black residents, with focus on those with higher rates of obesity and households with incomes below $25,000 into diabetes prevention programs; building the first mend program in baltimore city; increasing access to resources by developing BI-directional referral workflows and increasing social needs screening in the community and clinical settings; and improving the capacity of chws to serve people at risk for diabetes by developing and spreading a CHW-focused diabetes training, and developing a CHW reimbursement policy strategy. If awarded, these funds will help to reduce the risk of type 2 diabetes in 358,904 baltimore city residents through increased participation in diabetes prevention programs, increased implementation of diabetes prevention programs, increased contact with chws, increased the number of people screened and referred to community resources, and increased number of BI-direction communications between healthcare and community resources.
